OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Accidental Dental Injury

    Medical Necessity Criteria

Covered when ALL of the following are met

Primary conditions: 1) Injury results from extra-oral blunt force trauma to sound natural teeth and/or supporting hard and soft tissues (not due to chewing or biting); 2) The teeth were sound at the time of the accident (stable, functional, and free from decay, fracture, and advanced periodontal disease); 3) Treatment is generally rendered within 72 hours of the onset of the injury (exceptions for hospitalization-related delays); 4) Required clinical documentation is provided, including diagnostic pre- and post-trauma radiographs (properly oriented, labeled, and dated) such as full mouth, panoramic, or other appropriate radiographic images that show all existing and missing teeth in both arches; chart notes; intra-oral and facial photographs when appropriate; accident or emergency room report; and a letter from the treating dentist describing the accident, injuries, and proposed treatment plan.

Initial/emergency/palliative services do not require prospective review; retrospective review will assess appropriateness, coding, and payment based on prevailing dental standards.

    Definitive/Reconstructive Treatment

    Definitive/restorative coverage

Definitive treatment window: Definitive restorative and reconstructive treatment related to the accidental dental injury is considered when performed within 12 months of the date of injury.12 months

May include extraction; endodontic services; periodontic services (including soft tissue grafting); restorative services; implant services; fixed and removable prosthodontic services; and bone grafting.

Dental services that primarily treat a pre-existing or pre-accident dental condition are not considered appropriate under this policy unless the member's group contract specifically provides coverage for those services. The determination focuses on whether the condition requiring treatment existed prior to the accidental event rather than arising from the extra-oral blunt force trauma.

Services provided to address dental conditions that were present before the accident may be denied. When a procedure is performed in conjunction with, or in preparation for, a non-covered or denied service (including treatment of pre-existing conditions), related services may also be not covered or denied.

Retrospective review will be conducted after care is provided to evaluate reimbursement, documentation accuracy, coding, and medical/dental necessity based on prevailing dental community standards.

    • Retrospective review includes evaluation of reimbursement levels, documentation accuracy, coding accuracy, and adjudication of payment.
    Step Therapy

    Step Therapy

No step therapy requirements are specified for treatment of accidental dental injuries under this policy.

    Background

Accidental dental injuries result from extra-oral blunt force trauma to sound natural teeth and the supporting hard and soft tissues of the oral cavity, and exclude injuries caused by chewing or biting forces. Sound natural teeth are those that were stable, functional, and free from decay, fracture, and advanced periodontal disease at the time of the accident. Coverage for treatment of such injuries is based on clinical documentation that the injury was acute and the teeth were sound prior to the event.
