    POLICY UPDATE CHANGES

Criteria #11 was added specifying that LDCA/LDAA adjunctive therapy will not be considered for benefit when performed on the same date of service as scaling and root planing.

CDT codes D6080, D6180, D6280 were added to the coding list.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Indications for Coverage

Covered when ALL of the following are met:

Completion of definitive therapy: Local delivery is considered following completion of definitive non-surgical and/or surgical periodontal treatment (e.g., scaling and root planing).
Timing post-therapy: Procedure is performed 6 weeks to 6 months after scaling and root planing or periodontal surgery allowing adequate healing.6 weeks to 6 months
Site-specific clinical findings: Target tooth demonstrates an active/refractory site: periodontal pocket >=5 mm with spontaneous bleeding, bleeding on probing, or suppuration and history of increasing pocket depth with attachment loss.>=5 mm
    Documentation: A current (within 12 months), dated, post-periodontal maintenance and/or post-periodontal surgery 6-point periodontal charting is provided.within 12 months
Tooth selection: Only natural teeth with favorable periodontal outcomes will be considered.

    Not Medically Necessary / Experimental

Not covered or considered experimental when:

Use for bone regeneration: Use of LDCA/LDAA for regeneration of alveolar bone is considered experimental and investigational.
Generalized disease pattern: When a generalized pattern of periodontitis is diagnosed, LDCA/LDAA is not appropriate; more comprehensive intervention or surgical therapies should be considered.
Shallow pockets: LDCA/LDAA modalities are not appropriate for periodontal probing depths of less than 5 mm.<5 mm
Same-day SRP: LDCA/LDAA adjunctive therapy performed on the same date of service as scaling and root planing will not be considered for benefit.same date

LDCA/LDAA modalities are not appropriate for shallow periodontal pockets. Specifically, these local delivery therapies should not be used for sites with periodontal probing depths of less than 5 mm. In addition, when a generalized pattern of periodontitis is present, LDCA/LDAA are not indicated and more comprehensive or surgical interventions should be considered. Finally, if LDCA/LDAA is provided in conjunction with or in preparation for a non-covered or denied service, all related services may also be not covered or denied.

The use of LDCA/LDAA products for regeneration of alveolar bone has not been clinically tested and, therefore, is considered experimental and investigational. Use of LDCA/LDAA for alveolar bone regeneration is not a covered indication under this policy.

    Note

Code listing is informational — verify member benefits

The CDT codes listed in the policy are provided for informational purposes only and do not guarantee that a service is covered; verify the member's contract benefits and prior authorization requirements before billing or performing services.

    • Codes listed include: D4381, D4341, D4342, D4346, D4910, D4999, D6080, D6081, D6180, D6280 (informational only).
    • Inclusion or exclusion of codes does not constitute or imply member coverage or provider reimbursement—refer to member contract benefits.
    Prior Authorization

Use LDCA/LDAA only as adjunct after definitive periodontal therapy

LDCA/LDAA may be used only as adjunctive therapy after completion of definitive non-surgical and/or surgical periodontal treatment (for example, scaling and root planing) and typically only for isolated refractory sites.

    • Perform LDCA/LDAA only after definitive therapy is complete; most diseased sites respond to non-surgical therapy and do not require adjuncts.

    Background

Scaling and root planing (SRP) is the standard non‑surgical therapy for chronic periodontitis. Most periodontal sites respond to SRP and do not require adjunctive local delivery agents. LDCA/LDAA may be considered only as an adjunct for isolated, refractory sites after completion of definitive non‑surgical and/or surgical periodontal therapy, or during periodontal maintenance when localized recurrent inflammation persists.

    Definitions

    Refractory disease site definition
DefinitionA refractory disease site is an area with periodontal pocketing >= 5 mm accompanied by persistent signs of inflammation, spontaneous bleeding or bleeding on probing, suppuration, and/or increasing loss of clinical attachment.
Clinical contextConsidered after completion of definitive non-surgical and/or surgical periodontal treatment; may be identified during periodontal maintenance appointments.
Implication for treatmentIsolated refractory sites may be appropriate for LDCA/LDAA adjunctive therapy; multiple recurrent/residual sites may require more extensive periodontal modalities.
LDCA/LDAA definition — local delivery of chemotherapeutic or antimicrobial agents

Coverage CriteriaCoding & Clinical ThresholdsProvider Actions & Documentation RequirementsBackgroundDefinitions
Documentation purposeUsed to demonstrate active/refractory sites prior to LDCA/LDAA adjunctive therapy
  1. Consider LDCA/LDAA for isolated refractory pockets or during periodontal maintenance when localized recurrent inflammation persists.
  2. Documentation Required

    Submit a current (≤12 months) post-treatment 6‑point periodontal charting

Provide a current (within 12 months), dated, post-periodontal maintenance and/or post-periodontal surgery 6-point periodontal charting with the request or record to support consideration for benefit.

    • Charting must be post-periodontal maintenance and/or post-periodontal surgery and dated within the prior 12 months.
    Documentation Required

Document active/refractory site (pocket ≥5 mm with bleeding/suppuration)

Document that the target tooth demonstrates an active/refractory site: periodontal pocket ≥5 mm with spontaneous bleeding, bleeding on probing, or suppuration and a history of increasing pocket depth with attachment loss.

    • Clinical documentation must explicitly show pocket depth ≥5 mm plus bleeding on probing or spontaneous bleeding (or suppuration) and history of progression.
    Documentation Required

Medically compromised patients require case‑by‑case review and physician documentation

Use of LDCA/LDAA in medically compromised patients requires case‑by‑case review and must include supporting physician documentation.

    • Submit physician documentation when requesting LDCA/LDAA for medically compromised patients to allow individual review.
    Denial Risk

Same‑day LDCA/LDAA with scaling and root planing risks denial

Services performed on the same date as scaling and root planing are not considered for benefit and may be denied; do not perform LDCA/LDAA on the same date as SRP if seeking coverage.

    • Policy explicitly states LDCA/LDAA adjunctive therapy will not be considered for benefit when performed on the same date of service as scaling and root planing.
    Denial Risk

Performing LDCA/LDAA with non‑covered/denied services may make related services not covered

When LDCA/LDAA is performed in conjunction with or in preparation for a non‑covered or denied service, related services may also be not covered or denied; verify coverage of all planned procedures.

    • Note: the plan states that if any service is performed in conjunction with or in preparation for a non‑covered/denied service, all related services are also either not covered or denied.
Agents are placed subgingivally (local delivery) as adjunctive periodontal therapy
IndicationUsed as adjunctive therapy for isolated refractory pockets following completion of periodontal scaling and root planing or during periodontal maintenance when localized recurrent inflammation persists
LimitationsNot appropriate for pockets < 5 mm and not clinically tested for alveolar bone regeneration (considered experimental/investigational for that use).
