Local Anesthesia

Covered rules and exclusions for local anesthesia:

Administration of local anesthetic (traditional, electronic, buffered, etc.) is considered inclusive of all dental procedures and is not eligible for a separate benefit unless a specific plan allows coverage.

Regional and Trigeminal Division Block Anesthesia

Regional and trigeminal block anesthesia may not be a covered service.

Deep Sedation/General Anesthesia, Intravenous Moderate Sedation/Analgesia, Non-Intravenous Conscious Sedation

These procedures may be benefitted when performed in conjunction with covered dental services and when medical necessity criteria are met:

ALL of the following

Member meets at least ONE of
- The member is a child, up to the age defined by contract, with a dental condition requiring repairs of significant complexity (examples: multiple restorations, pulpal therapy, extractions or combinations of such procedures).
- The member has physical, intellectual, or medically compromising conditions for which dental treatment under local anesthesia cannot be expected to provide a successful result (examples include intellectual disabilities, cerebral palsy, epilepsy, severe cardiac problems, hyperactivity). Requires appropriate medical documentation.
- The member is extremely uncooperative, fearful, unmanageable, anxious, or uncommunicative with complex dental needs that should not be postponed and for whom lack of treatment may result in oral pain, infection, loss of teeth, or increased oral morbidity.
- Local anesthesia is ineffective for the member (reasons such as acute infection, anatomic variations or allergy). Failed attempts of local anesthesia administration must be documented and submitted for review.
- The member has sustained extensive oral-facial and/or dental trauma for which treatment under local anesthesia would be ineffective or compromised.
Medical conditions requiring general anesthesia or moderate sedation must be supported by documentation submitted by the oral surgeon, dentist and/or physician administering the anesthesia.
The level of anesthesia is determined by the documented effect on the patient's central nervous system, not solely by the route of administration.
Time of general anesthesia begins when the dentist starts administering the sedative agent and non-invasive monitoring protocol; the dentist must remain in constant attendance until completion.
Services are complete when the patient can be safely left under observation of trained personnel and the dentist may leave the operatory.
Office anesthesia in excess of 60 minutes for any dental or surgical procedure requires a written rationale/documentation explaining the necessity, including all associated radiographic images, progress notes, operative report and a complete anesthesia record indicating start and stop times of incremental drug administration.

Inhalation of Nitrous Oxide/Analgesia

Nitrous oxide may be benefitted when appropriate in conjunction with covered dental services and is indicated/contraindicated as follows:

Nitrous oxide may be indicated for
- Ineffective local anesthesia administration
- Treatment of dental anxiety
- Special needs patients
- Individuals who are uncooperative or behaviorally challenged

Monitoring and Documentation Requirements

Required monitoring and documentation during anesthesia:

A dentist or appropriately trained individual must remain in the operatory during active dental treatment to continuously monitor the patient's vital signs until discharge criteria to recovery area are met.

Monitoring must include observation of proper oxygenation (including mucosal color) and continuous evaluation of skin or blood; oxygen saturation by pulse oximetry is appropriate and necessary.

Ventilation: chest movement must be continually observed; respirations must be verified and documented by appropriately trained personnel.

Circulation: blood pressure and heart rate must be evaluated pre-operatively, intra-operatively and postoperatively and properly documented.

Documentation: maintain a complete anesthesia record including names of drugs administered, time and dosages, and all physiological parameters.

Background and Definitions

The policy describes a continuum of sedation levels — anxiolysis, analgesia, intravenous moderate sedation, and deep sedation/general anesthesia — defined by the effect of agents on the central nervous system and aligned with ADA/AAP/AAPD/ASA guidance. Dental professionals administering anesthesia must be appropriately educated, trained, and licensed; for advanced anesthetics (IV or inhalation), additional individuals with Advanced or Basic Life Support credentials and, where required by state, current equipment inspection, sedation license and CPR certification must be present.

Monitoring and facility readiness requirements include continuous in‑operatory monitoring by the dentist or an appropriately trained individual until discharge criteria are met, observation of oxygenation (including mucosal color), use of pulse oximetry, continuous ventilation and circulation assessment (pre/intra/post‑op blood pressure and heart rate), and maintenance of a complete anesthesia record listing drugs, times, dosages and physiologic parameters. The dentist is responsible for emergency management, appropriate equipment, drugs, staff and office protocols.
