Coverage Criteria for Urine Culture Testing

Reimbursable indications and non-reimbursable situations

Urine culture testing (with isolate identification and antibiotic susceptibilities if applicable) may be reimbursed when any of the following specific situations apply; otherwise it is not reimbursable.

Reimbursable situations: Pregnant individuals; Asymptomatic individuals undergoing urological interventions which breach the mucosa (pre-procedure); Individuals exhibiting at least one sign or symptom of possible UTI or bacteriuria (see Note 1); To assess pyelonephritis; Other specified exceptions per plan or pathogen panel guidance; When isolate identification and antibiotic susceptibilities are applicable.

Non-reimbursable situations: For all other instances of asymptomatic urinary tract infection or asymptomatic bacteriuria not described above, urine culture testing (with isolate identification and antibiotic susceptibilities if applicable) is not reimbursable; Follow-up urine culture testing for an uncomplicated urinary tract infection in individuals who show evidence of clinical resolution is not reimbursable; Urine culture testing is not reimbursable as part of assessment or prognosis of prostate biopsy or as part of initial screening for asymptomatic prostatitis.

Urine culture testing (including isolate identification and antibiotic susceptibilities when applicable) is not reimbursable for routine evaluation of asymptomatic bacteriuria except where specific exceptions apply. Permitted exceptions include testing for pregnant individuals and for asymptomatic patients undergoing urological interventions that breach the mucosa when performed prior to the procedure. Additionally, urine culture is allowed when an individual is exhibiting at least one recognized sign or symptom of urinary tract infection or bacteriuria (see Note 1) or when assessing suspected pyelonephritis. Testing performed solely as part of prostate biopsy assessment or as initial screening for asymptomatic prostatitis is explicitly not reimbursable.

Follow-up urine culture testing after completion of therapy for an uncomplicated urinary tract infection is considered not reimbursable when the individual shows evidence of clinical resolution. In other words, routine post-treatment cultures for uncomplicated UTI are not covered unless a different covered indication or clinical concern is documented (for example persistent or recurrent symptoms or another exception described in this policy).

Pregnancy

Covered when ALL of the following are met:

Pre-procedural testing for urological interventions that breach mucosa

Covered when ANY of the following pre-procedural conditions apply:

Pre-procedural testing for urological interventions that breach mucosa: Asymptomatic individual is scheduled for a urological intervention which will breach the mucosa and urine culture testing (with isolate identification and antibiotic susceptibilities if applicable) is ordered prior to the procedure.

Pre-procedure testing applies to interventions that breach the mucosa; check plan-specific prior authorization requirements

Symptomatic individuals

Covered when ONE OR MORE of the following signs or symptoms are present (see Note 1):

Symptomatic individuals: Individual exhibits at least one sign or symptom of possible UTI or bacteriuria (examples include fever; urgency; increased frequency; feeling need to urinate despite empty bladder; dysuria; suprapubic tenderness; pyuria; hematuria; flank pain; cloudy urine; nausea; vomiting; chills; night sweats; change in urine smell; pelvic pressure; abnormal urinalysis findings).

Assessment of pyelonephritis

Covered when ALL of the following are met:
