OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

All other indications are considered experimental/investigational and not medically necessary.

    inv-02: Authorization durations

    Authorization durations

Initial authorization duration: Authorization may be granted for 6 months for diabetic macular edema, diabetic retinopathy, neovascular (wet) age-related macular degeneration, macular edema following retinal vein occlusion, and retinopathy of prematurity (Eylea and biosimilars only).6 months

Documentation of clinical response required for reauthorization.

    inv-03: Dosing and quantity limits

Dosing / quantity limits by product and indication

Standard intravitreal dosing (most products): Initial: 2 mg per affected eye every 4 weeks for specified loading doses (commonly 3 or 5 doses depending on product/indication). Maintenance: 2 mg per affected eye every 4 weeks (approximately every 25 days, monthly).2 mg q4w

    Product-specific variations described below.

Eylea HD dosing: Eylea HD initial: 8 mg per affected eye every 4 weeks (+/- 7 days) for the first 3 (or 3–5 for some indications) doses. Maintenance: 8 mg per affected eye every 8 to 16 weeks (+/- 7 days); may be dosed as frequently as every 4 weeks (+/- 7 days) to maintain response.8 mg

Higher-dose regimen is specific to Eylea HD.

Retinopathy of prematurity dosing (Eylea): Eylea for ROP: 0.4 mg per affected eye; may be repeated up to a total of 3 doses, at least 10 days apart.

All indications not listed as FDA-approved or included as compendial uses in this policy are considered experimental/investigational and not medically necessary. The FDA‑approved indications for Eylea and the listed aflibercept products are limited to diabetic retinopathy, diabetic macular edema, neovascular (wet) age-related macular degeneration, macular edema following retinal vein occlusion, and retinopathy of prematurity (Eylea and biosimilars); Enzeevu is indicated for neovascular (wet) AMD. Use outside these listed indications will be denied as investigational.

This policy is written to comply with Tennessee Code Annotated §56-7-2352. Per that statute, off‑label uses of FDA‑approved drugs may be considered for coverage only when the off‑label indication is recognized by one of the statutorily recognized standard reference compendia or is supported in the published peer‑reviewed medical literature; documentation meeting those requirements must be provided for consideration of coverage.

Use of aflibercept products for indications other than those explicitly listed as FDA‑approved or recognized compendial uses in this policy is considered not medically necessary. Requests for treatment of unlisted indications will be reviewed and are expected to be denied unless the indication meets the compendial or peer‑reviewed literature criteria required by state statute.
