Coverage Criteria for Remote Electrical Neuromodulation (REN)
Coverage for chronic migraine
Covered when the following high-level conditions are met:

Nerivio (remote electrical neuromodulation) is contraindicated in patients with uncontrolled epilepsy and in patients with an active implantable medical device (for example, pacemaker, hearing aid implant, or any implanted electronic device). The device has not been evaluated in certain populations including patients with congestive heart failure, severe cardiac or cerebrovascular disease, pregnancy, or children younger than 8 years.
If services are determined to be not medically necessary or are otherwise non-covered benefits under the member’s contract, the provider may not bill or collect payment from the member unless the member was informed in advance and provided written agreement to pay out-of-pocket. Benefits and eligibility are governed by the member’s subscriber agreement, benefit booklet, or employer agreement, which supersede this medical policy.

Remote electrical neuromodulation (REN, e.g., Nerivio) is a wearable, patient‑controlled peripheral nerve stimulator applied to the upper arm that induces conditioned pain modulation and is used as a nonpharmacologic option for acute treatment and, in more recent clearances, for preventive treatment of migraine in appropriate age groups.
