OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medicare Advantage coverage
    Not medically necessaryCommercial Product coverage

    Coverage Criteria

For Medicare Advantage Plans, polygenic risk score testing used to provide diagnostic, prognostic, therapeutic, or future risk assessment results is not covered.

For certain self-funded Commercial groups that exclude expanded biomarker-testing coverage related to the Rhode Island state mandate, applicable genetic testing services may be excluded by contract. The member’s Benefit Booklet determines whether customized benefit coverage applies.

For Commercial Products, polygenic risk score testing used to provide diagnostic, prognostic, therapeutic, or future risk assessment results is not medically necessary because the evidence is insufficient to determine that the technology improves net health outcomes.

The testing lacks demonstrated clinical utility. Published data may be absent or extremely limited; the test’s role in the clinical pathway may be unclear; its effect on management and health outcomes may be uncertain; decision thresholds may not be established; and test results may not provide a clinically meaningful improvement over outcomes achieved without testing. Accordingly, the evidence is insufficient to establish an improvement in net health outcomes.

    Prior Authorization Required

Prior authorization is required for every unlisted genetic testing CPT code to identify the service being rendered and determine whether it is covered or not medically necessary.

    Documentation Required

    Ordering Physician Documentation

The ordering physician must provide the information and documentation needed to evaluate the unlisted service, its clinical appropriateness, and coverage or medical-necessity status.

    Prior Authorization

    Authorization Responsibility

Only the ordering physician may handle authorization, appeals, and other administrative processes related to prior authorization or medical necessity.

    Background

A polygenic risk score uses genetic information to estimate an individual’s risk of developing disease. Testing may be used for diagnostic, prognostic, therapeutic, or future disease-risk assessment; however, the policy determines that evidence is insufficient to establish improved net health outcomes for these uses.

    Not Covered Services

Polygenic risk score testing performed for diagnostic, prognostic, therapeutic, or future risk assessment is not covered for Medicare Advantage Plans. For Commercial Products, the same testing is not medically necessary because evidence is insufficient to establish improved net health outcomes.

    Prior Authorization

    PROVIDER ACTIONS

Ordering physicians must obtain prior authorization for unlisted genetic testing and remain directly involved in the authorization, clinical documentation, appeals, and related administrative processes.
