    Coverage Criteria

    Covered psychotropic pharmacogenomics panel

Covered for Medicare Advantage Plans and Commercial Products.

Covered test: Psychotropic Pharmacogenomics Gene Panel (Mayo Clinic), billed under CPT 81479, is covered.

No additional coverage criteria are stated.

    Excluded mental-health genetic tests

Not covered for Medicare Advantage Plans and not medically necessary for Commercial Products because the evidence is insufficient to determine that the technology improves the net health outcome.

Excluded or not medically necessary test

    • Cytochrome P450 1A2 Genotype (Mayo Clinic).
    • Genomind Professional PGx Express CORE (Genomind, Inc.).
    • Neuropharmagen (Precision Genetics, Inc.).
    • OptimalRx Plus (Dynacare Plus).
    • Psych HealthPGx Panel (RPRD Diagnostics).

    Clinical documentation requirements

    Documentation must establish clinical appropriateness.

    Documentation requirements

    • The individual has a diagnosis for which pharmacologic therapy is reasonable and necessary, and the drug or drugs being considered are reasonable and necessary for treatment of the individual's diagnosis.
    • The clinician has made an initial personalized treatment decision based on the diagnosis, other medical conditions, other medications the individual is taking, professional judgment, clinical and basic science pertinent to the drug, including mechanism of action and side effects, the individual's past medical history, pertinent family history, and the individual's preferences and values.
    • The provider performing the service maintains a record identifying the drug or drugs being considered and the indication or indications for use.

The Psychotropic Pharmacogenomics Gene Panel (Mayo Clinic), billed under CPT 81479, is covered for Medicare Advantage Plans and Commercial Products. The policy does not state additional indication-specific coverage criteria for this panel.

The following tests are not covered for Medicare Advantage Plans and are not medically necessary for Commercial Products: Cytochrome P450 1A2 Genotype (Mayo Clinic), Genomind Professional PGx Express CORE, Neuropharmagen, OptimalRx Plus, and Psych HealthPGx Panel.

For Commercial Products, the listed tests are considered not medically necessary because the evidence is insufficient to determine that the technology improves the net health outcome.

Evidence is insufficient to establish improved net health outcomes for genetic testing used to assess the risk of mental illness. No studies were identified showing that testing changed clinical management or affected health outcomes. Evidence for Neuropharmagen-guided treatment in adults with Major Depressive Disorder is also insufficient; the available trials had important limitations in design, conduct, consistency, and precision.

    Covered Indications

Psychotropic pharmacogenomic testing for mental health treatment management

Covered for mental health treatment management under CPT 81479 for Medicare Advantage Plans and Commercial Products.

Covered indication: Psychotropic pharmacogenomic testing for mental health treatment management using the Psychotropic Pharmacogenomics Gene Panel is covered.

No additional indication-specific criteria are stated.

    Eligibility Requirements

When relevant to the clinician's personalized treatment decision, pertinent family history must be considered and documented in the medical record.

    Provider Actions and Authorization

    Prior Authorization

Prior Authorization for Unlisted Genetic Testing

Prior authorization is required for every unlisted genetic testing CPT code to determine the service being rendered and whether it is covered or not medically necessary.

    • For services without a specific CPT code, use unlisted CPT code 81479.
    • Claims for an entire panel must use the applicable unlisted CPT code, even if specific CPT codes exist for some panel components.
    Prior Authorization

    Ordering Physician Controls Authorization

Only the ordering physician may participate in prior authorization, appeals, and other administrative processes related to prior authorization or medical necessity.

    • The ordering physician must provide and control the clinical-appropriateness documentation.
    • A laboratory, third party, or laboratory-affiliated representative may not obtain authorization or participate on the ordering physician’s behalf.

    Background

Pharmacogenomic testing is intended to inform medication selection, dosing, treatment response, drug metabolism, or adverse-event risk in mental health care. Genetic variation generally explains only a modest portion of treatment-response variability because outcomes are also influenced by alternate metabolic pathways and individual- and disease-related factors. Clinical validity alone does not establish clinical utility; evidence should show that using the test to guide management improves outcomes compared with standard care or no testing.

    Definitions

    Laboratory-developed test (LDT)
DefinitionA laboratory-developed test is developed and validated by a clinical laboratory and offered as a laboratory service.
Regulatory standardLaboratory-developed tests must meet the general regulatory standards of the Clinical Laboratory Improvement Amendments (CLIA).
Laboratory requirementLaboratories that offer laboratory-developed tests must be licensed by CLIA for high-complexity testing.
    Net health outcome
DefinitionNet health outcome is the balance of benefits and harms when a test is used to manage a condition.
ComparisonThe net health outcome is better when the balance of benefits and harms is better with the test than with another test or no test.

    Not Covered Services

The following tests are not covered: Psych HealthPGx Panel, Genomind Professional PGx Express CORE, Neuropharmagen, OptimalRx Plus, and Cytochrome P450 1A2 Genotype. For Medicare Advantage Plans, these services are not covered; for Commercial Products, they are not medically necessary.

    Denial Risk

Unauthorized Unlisted Testing May Be Denied

Unlisted genetic testing performed without the required prior authorization will be denied. If a laboratory provides an unauthorized laboratory service, the denial is the financial responsibility of the participating laboratory and the service may not be billed to the member.

    • Claims may also be denied when the authorization process was not followed or clinical appropriateness was not established.
    • BCBSRI may take severe action, up to and including termination from the provider network, when a laboratory or third party supports the authorization process in violation of this policy.
    Denial Risk

Laboratories May Not Participate in Authorization

Laboratories and third parties may not obtain clinical authorization, prepare clinical-appropriateness documentation, facilitate any portion of the authorization process, or participate in an appeal on behalf of the ordering physician.

    • A physician or provider may not use a laboratory representative or anyone with a relationship to a laboratory or third party for these purposes.
    • This prohibition applies to authorization, documentation, appeals, and related administrative processes.

    Verify Benefit and Contract Limitations

Coverage and benefits may vary by group and contract. Some self-funded groups may have a contract exclusion for expanded biomarker-testing coverage related to the Rhode Island mandate, R.I.G.L. §27-19-81.

    • Consult the applicable Benefit Booklet, Evidence of Coverage, Subscriber Agreement, or member certificate.
    • For self-funded groups, verify whether the plan excludes expanded biomarker-testing coverage.
    • Refer to the applicable coding section and related policies for services covered with prior authorization, not medically necessary, or excluded by contract.
    Prior Authorization

    Provider Actions

The ordering physician alone must obtain prior authorization and participate in authorization, appeal, and related administrative processes; laboratories, third parties, and laboratory-affiliated representatives may not act on the physician’s behalf.

    • The ordering physician is responsible for submitting the clinical-appropriateness documentation and managing any appeal.
    • Do not delegate or route authorization-related activities through the performing laboratory or another third party.
Potential improvementA net health outcome can improve when individuals receive correct or more effective therapy, or avoid unnecessary therapy or testing.
