Coverage Criteria
Covered when ALL of the following are met
Oral nutritional formulas, special medical formulas, and low-protein food products are non-covered and a contractual exclusion for all Commercial Products when the specified medical criteria in this policy are not met. The policy applies to Commercial products only, and coverage is limited to physician-ordered use for the conditions and indications listed in the Coverage Criteria (e.g., malabsorption from Crohn's disease, ulcerative colitis, gastroesophageal reflux, chronic intestinal pseudo-obstruction, inherited diseases of amino acids and organic acids, and select infant milk/soy protein allergy cases).
If services or products are determined to be not medically necessary or do not meet the policy criteria, they may not be covered and providers may not bill or charge the member unless the member has been informed and has agreed in writing in advance to pay. For Commercial Products, prior authorization is recommended and obtained via the insurer's online authorization tool for participating providers; non-participating providers may use the provided preauthorization form. For member reimbursement after preauthorization approval, members must follow the claims instructions and submit required documentation as specified by BCBSRI.

Prior Authorization Recommended
Prior authorization recommended for Commercial products. Participating providers must obtain authorization via Blue Cross & Blue Shield of Rhode Island's online authorization tool before dispensing low protein foods, oral nutritional formulas, or special medical formulas for home use whenever coverage criteria may apply.

Provider Actions and Documentation
Provider responsibilities: confirm member eligibility and commercial product coverage criteria before ordering; document the medical indication tying the request to an approved condition (e.g., PKU, tyrosinemia, Crohn's disease, ulcerative colitis, gastroesophageal reflux, chronic intestinal pseudo-obstruction, inherited amino/organic acid disorders, or infant milk/soy protein allergy with failure to thrive and/or blood in stools). Submit prior authorization requests through the online authorization tool (or the Preauthorization Form for Oral Nutrition for non-participating providers). Maintain copies of the physician order and supporting clinical documentation in the patient record to support medical necessity.

Member Reimbursement After Preauthorization
Member reimbursement after preauthorization: members may be reimbursed for eligible oral nutritional products and low protein foods after obtaining preauthorization. The member must submit itemized receipts (with special foods highlighted or circled) and the reimbursement form within one year from the date of purchase to the address below.

Non-Covered Requests When Contract Exclusion Applies
Requests for low protein foods, oral nutritional formulas, or special medical formulas are not covered when the member does not meet the specific contractual criteria listed for Commercial products. If the diagnosis/clinical criteria in this policy are not met, the request is a contract exclusion and should not be authorized or billed to the plan.

Definitions
Background and Rationale
Non-prescription oral nutritional formulas and special medical formulas are used for home management of malabsorption and certain metabolic or inborn errors where modified diets or specialized formulas are medically necessary. Examples include oral nutritional liquids (e.g., Ensure, Boost) and special medical formulas (e.g., Neocate) when ordered by a physician to treat malabsorption caused by Crohn's disease, ulcerative colitis, gastroesophageal reflux, chronic intestinal pseudo-obstruction, or for inherited diseases of amino acids and organic acids. Low-protein food products are indicated for inherited amino acid and organic acid disorders such as phenylketonuria (PKU), tyrosinemia, homocystinuria, maple syrup urine disease, propionic aciduria, and methylmalonic aciduria. The policy also recognizes use of formula for milk/soy protein allergy in infants (0–12 months) presenting with failure to thrive and/or blood in the stools when ordered by a physician.
