Medically necessary criteria
Air or water ambulance is considered medically necessary when ALL of the following conditions are met (logical grouping preserved as listed):
applies to transfers between hospitals lacking required facilities
Specific benefit limitations for ambulance services vary by contract. Always refer to the member's applicable Benefit Booklet, Evidence of Coverage, Subscriber Agreement, or employer agreement to determine covered ambulance benefits and any contract-specific limits.
If a service is not covered under the member's subscriber agreement or employer agreement, it is excluded and the plan documents will supersede this policy for determinations of benefits and eligibility.
If services are determined to be not medically necessary or are non-covered benefits per the member's contract, providers may not charge the member for those services unless the provider clearly informed the member and the member provided written agreement in advance to accept financial responsibility. Check participation agreements and plan-specific rules before billing members.
