Clinical Background
Meibomian gland dysfunction (MGD) is the most common cause of evaporative dry eye and becomes more prevalent with age, affecting millions of individuals. Thermal pulsation devices deliver controlled localized heat to the inner eyelid surface combined with intermittent pulsatile massage to the outer eyelid surface to facilitate expression of lipid from dysfunctional meibomian glands. These devices are an alternative to warm compresses, manual expression, eyelid hygiene, and medications, but available randomized trials and systematic reviews provide mixed results with low to very low certainty and do not demonstrate sufficient net health outcome improvement to support coverage.
