    POLICY UPDATE CHANGES

Added CPT/HCPCS code 83884 to the procedure codes list.

Title changed from 'Serum Biomarker Testing for Multiple Sclerosis and Related Neurologic Diseases' to current title and reimbursement information unchanged.

Reimbursement criteria for NMOSD/MOG-EM clarified to require that all listed conditions be met for serum AQP4-IgG and MOG-IgG testing to be reimbursable.

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria for Biomarker Testing

    Coverage criteria for biomarker testing

Reimbursement is conditional and limited to specific clinical scenarios described below.

CSF oligoclonal band analysis for MS

    • For individuals with atypical clinical, laboratory, or imaging features
    • For individuals belonging to a population in which MS is less common (e.g., children, older individuals)
    • For individuals with an atypical clinically isolated syndrome (including primary progressive or relapsing-remitting courses)
    • When clinical or imaging evidence is insufficient for diagnosis

    Serum AQP4-IgG and MOG-IgG testing for NMOSD/MOG-EM

    • The individual has monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof
    • Radiological or electrophysiological findings compatible with central nervous system (CNS) demyelination
    • At least one high‑risk feature present

      • Extensive spinal cord lesion or atrophy on MRI, or large confluent T2 brain lesions
      • Belongs to a higher‑risk population (e.g., pediatric)
      • Prominent papilledema, papillitis, or optic disc swelling during acute optic neuritis
      • Histopathology with primary demyelination and intralesional complement and IgG deposits, or prior diagnosis of pattern II MS
      • Neutrophilic CSF pleocytosis
      • Simultaneous bilateral acute optic neuritis

    Other serum assays

    • ELISA, Western blot, immunohistochemistry, or any other serum assays to test for NMOSD or MOG-EM are not reimbursable

    Other CSF biomarker tests

    • All other CSF biomarker tests, including AQP4-IgG or MOG-IgG for diagnosis of MS, NMOSD, or MOG-EM, are not reimbursable

    Default stance

    • In all other situations, serum biomarker tests for multiple sclerosis are not reimbursable

    Documentation and Claim Review Expectations

    Documentation Required

    Documentation and Claim Review Expectations

Providers are responsible for submitting accurate and complete documentation for services performed. Claims must be coded using valid, HIPAA-approved code sets and in accordance with industry-standard coding guidelines (for example: Uniform Billing Editor, AMA CPT and CPT Assistant, HCPCS, ICD-10-CM/PCS, NDCs, DRG guidance, CMS National Correct Coding Initiative and CCI edits). Claims are subject to code-edit protocols and may be reviewed for consistency with benefit coverage, provider contract terms, medical policies, clinical payment and coding policies, and automated coding/software logic. Upon request, providers should supply additional supporting documentation to facilitate claim review and adjudication.

    • Submit claims with accurate code combinations from HIPAA-approved code sets (CPT, HCPCS, ICD-10, NDC, etc.).
    • Follow industry standard coding guidance (AMA CPT, Uniform Billing Editor, CMS NCCI/CCI edits, DRG rules).
    • Claims may be reviewed against benefit documents, provider contracts, medical and coding policies, and coding software logic.
    • If requested, promptly provide additional documentation to support the claim.
    • BCBSNM retains discretion to interpret and apply this policy; plan documents and provider contracts govern in case of conflict.

    Plan Definitions and Covered Assays

Plan reimbursement criteria — lab management reimbursement criteria
DefinitionPlan reimbursement criteria: Lab management reimbursement criteria implemented by the Plan; not all requirements apply to each product and providers should review Plan documents for eligible coverage.
ScopeApplies to cerebrospinal fluid (CSF) and serum biomarker testing used in diagnosis of MS, NMOSD, and MOG-EM when specific clinical criteria are met.
Provider responsibilityProviders are urged to review Plan documents for eligible coverage for services rendered.
Operational noteNot all requirements apply to each product — check member plan for applicability.
