    Coverage Criteria for Residential Treatment

    Covered residential treatment criteria

Covered when ALL of the following are met

Admission prerequisites: Member has clearly documented history of excessive use of alcohol or other psychoactive substances and a physician certifies a substance-related disorder (ICD diagnosis code range) and placement decisions use ASAM or LOCUS criteria.
Facility qualifications: Facility meets Blue Cross/BCN substance use disorder facility requirements, is licensed by Michigan as an appropriate facility type, uses a multidisciplinary team and participates with Blue Cross/BCN or local Blue plan as applicable.
Program service and staffing requirements: 24/7 supervision; nursing onsite or on-call within 60 minutes; psychiatrist on call; psychiatrist onsite minimum two days/week; attending psychiatrist face-to-face or virtual evaluation within 48 hours of admission; initial individualized plan of care developed within 72 hours and a more robust plan by end of week 2; weekly plan updates; individual therapy twice weekly; group therapy minimum 12.5 hours/week with 60–90 minute sessions, at least two psychological group sessions and remaining sessions may be expressive or psychoeducational; medication management by a board-certified psychiatrist or qualified advanced practice clinician under attending psychiatrist supervision at least twice weekly until stable then at least weekly; ongoing medical services and multidisciplinary integrated treatment focused on rehabilitation and community reintegration.
Detoxification criteria: Subacute detoxification is covered when provided in conjunction with a 24-hour residential rehabilitation program or as part of an outpatient rehabilitation program; acute detoxification requiring inpatient medical hospital care is reserved for members with severe, life‑threatening physiologic dysfunction during withdrawal.

The following services are not covered under the substance use disorder residential treatment benefit. Providers must obtain prior authorization when required and confirm services meet medical necessity; failure to obtain prior authorization will result in nonpayment for admission or services. Specific exclusions include: admissions or services when prior authorization was not obtained; staff consultations required only by facility rules; marital counseling; admissions or continued stays initiated or prolonged solely for socioeconomic, domestic, or housing reasons; services provided during any portion of an admission that occurs before the member’s coverage effective date; and services provided when the member is not engaged or participating after a reasonable period of time.

Additional noncovered items include extra charges for group or individual psychotherapy lasting more than 90 minutes, repeated or redundant services when there is no expectation of change in the member’s condition, services by nonparticipating facilities (for facilities located in Michigan or not participating with the local Blue plan for out-of-state facilities), services not focused on improving the member’s functioning, and services primarily intended to maintain gains from another program.

Programs and service types excluded from separate payment include residential programs that substitute for unavailable community supportive living, therapeutic boarding schools, milieu therapies (for example wilderness programs, supportive houses, group homes), domiciliary foster care, custodial care, programs for sex offenders or perpetrators of violence, confinement when no medical treatment is required, dispensing methadone or urine testing unless paired with therapy/counseling/psychological testing, transitional living centers (halfway/three-quarter-way houses), private rooms or apartments, and nonmedical activities (for example art, equine, music, dance, yoga, ropes courses, guided imagery) — these nonmedical services may be included in program payment but are not payable separately.

Continued stay in a residential rehabilitation program is not covered without documentation that supports ongoing medical necessity. The policy specifically excludes continued stays lacking a thorough evaluation, an individualized treatment plan, and convincing justification for the continued level of care.

Other conditions rendering continued stay not medically necessary include member failure to engage or participate after a reasonable period and provision of services or continued stay after the physician has discharged the member from the program. Providers should document weekly updates to the individualized plan of care and clearly justify the clinical need for each continued day of residential care to avoid denial of coverage.

    Provider Requirements and Prior Authorization

    Prior Authorization

    Prior Authorization Required

Residential treatment for substance use disorders requires prior authorization and requests are reviewed against Blue Cross Behavioral Health medical necessity criteria. If the member's behavioral health care is managed by Blue Cross Behavioral Health, providers should follow the Blue Cross Behavioral Health prior authorization process and criteria available in the Behavioral Health Services section of the Services That Need Prior Authorization webpage at bcbsm.com. For members whose care is managed by an entity other than Blue Cross Behavioral Health, refer to the list of Utilization Management and Care Management Opt-outs.

    • Prior authorization is required for residential treatment for substance use disorders.
    • If prior authorization is not obtained, participating (in‑network) facilities cannot bill the member; nonparticipating (out‑of‑network) facilities may require member payment.
    Documentation Required

    Detoxification Site Requirements

Acute detoxification must be provided in an inpatient medical hospital setting when withdrawal causes severe physiological dysfunction that threatens life or significant bodily functions. Subacute detoxification is covered when provided by a Blue Cross‑approved hospital‑based or freestanding substance use disorder treatment facility, either in conjunction with a 24‑hour residential rehabilitation program or as part of an outpatient rehabilitation program. Additional charges for subacute detoxification (for example, for mild withdrawal) are not covered unless the member requires active daily medical management.

    Background and Context

Detoxification for substance use disorders is described in two clinical contexts. Acute detoxification refers to withdrawal that threatens life or significant bodily function and therefore must be managed in an inpatient medical hospital setting with active medical management.

Subacute detoxification describes physiologic dysfunction during withdrawal that is not life threatening but still requires monitoring and coordinated rehabilitation. Subacute detoxification may be provided within a 24-hour residential rehabilitation program or as part of an outpatient rehabilitation program when clinically appropriate.

    Level of Care — Residential

Residential — level of care criteria

Residential level of care is met when ALL of the following top-level criteria are satisfied:

Setting and intensity: Care is delivered in a licensed residential program that provides live‑in services 24 hours a day, seven days a week, and that accommodates members who require supervision around the clock and who cannot safely be treated in a less restrictive setting.
Admission and placement determination: A physician certifies a substance‑related disorder and placement decisions use standardized criteria such as ASAM or LOCUS to determine severity and intensity of services required.
Facility licensing and program model: Facility is licensed by the state of Michigan in an appropriate category (e.g., child‑caring institution, adult foster care, or other applicable health care facility) and uses a multidisciplinary treatment team with an identified medical director.

    Treatment Modalities and Services

    Detoxification (acute vs subacute)

Detoxification pathways — follow ONE of the pathways below depending on clinical severity:

Detoxification pathway: Acute detoxification: Member exhibits severe physiologic dysfunction during withdrawal that threatens life or significant bodily functions; care must be provided in an inpatient medical hospital setting.
Detoxification pathway: Subacute detoxification: Member may suffer physiologic dysfunction during withdrawal but withdrawal is not life‑threatening; subacute detoxification is provided within residential or outpatient rehabilitation programs and covered when paired with an approved program.

    Documentation Required

Plan of Care and Weekly Documentation

An individualized plan of active substance use disorder treatment and residential living support must be developed in a timely manner: the initial plan of care should be developed within 72 hours of admission and a more comprehensive plan by the end of week 2. Each member's plan of care must be updated at least weekly and include, at minimum, a weekly face‑to‑face or audio‑visual meeting with the patient and caregiver/family (unless clinically inappropriate), medication management (psychiatrist or qualified prescriber) at least twice weekly until stable then at least weekly, ongoing medical services for comorbid conditions, integrated multidisciplinary treatment with community coordination, individual therapy twice weekly, and a minimum of 12.5 hours per week of group therapy (60–90 minute sessions, with at least two psychological process groups).

    • Initial plan of care: within 72 hours of admission.
    • Comprehensive plan: by end of week 2.
    • Plan updates: at least weekly, documenting participation or valid reasons for non‑participation.
    • Minimum plan elements: weekly face‑to‑face/AV meetings; medication management frequency; ongoing medical services; multidisciplinary integrated treatment; individual therapy twice weekly; group therapy ≥12.5 hours/week with required session structure.
    Denial Risk

    Continued Stay Documentation

Continued stay in a residential rehabilitation program must be supported by thorough evaluation, an individualized treatment plan, documentation of ongoing participation (or a clinically valid reason why the member cannot participate), and convincing clinical justification for continued services. Continued stay without documentation of a thorough evaluation, individualized treatment and convincing justification is not covered and may result in denial of payment. Services provided after a physician has discharged the member from the treatment program are not covered.

    • Continued stay requires documented thorough evaluation and individualized treatment plan.
    • If the member cannot participate, documentation must explain why and provide a plan for engagement or justification for continued stay.
    • Continued stay without required documentation is not covered and may be denied.
    • Services after physician discharge are not covered.
