    Coverage and Medical Necessity Criteria

Medical necessity for residential psychiatric treatment

Covered when ALL of the following are met:

Facility licensing and participation: Facility is licensed by the state (e.g., child-caring institution, adult foster care or other appropriate license) and participates with Blue Cross/BCN (for Michigan facilities) or the local Blue plan (for out-of-state).
Clinical need: Member requires psychiatric treatment longer term than traditional inpatient and more intensive than outpatient, does not require inpatient intensity but requires 24/7 supervision because they cannot be safely treated in a less restrictive setting.
Multidisciplinary care: Use of a multidisciplinary treatment team and ability to provide room, board, supervision, nursing care, psychiatric services, individual and group therapy, educational services for children, and age-appropriate resources for geriatrics.

    Program operational and clinical requirements

Required operational and clinical program elements:

Psychiatric evaluation timing: Face-to-face or audio-visual evaluation by the attending psychiatrist within 48 hours of admission.48 hours
Psychiatrist presence: A psychiatrist must be onsite a minimum of two days per week and available on call; a psychiatrist on call is required 24/7.2 days/week
Nursing coverage: Nursing care must be onsite or on call and no more than 60 minutes away, available 24/7.60 minutes
Plan of care timing and updates: Initial plan of care developed within 72 hours of admission with a more robust plan by the end of week 2; individualized plans updated at least weekly and must include weekly face-to-face/virtual meeting with member and caretaker when clinically appropriate.

The policy excludes a range of programs and services from payment. Wilderness programs, therapeutic boarding schools, transitional living centers (halfway and three-quarter-way houses), milieu therapies, domiciliary foster care, custodial care, private rooms or apartments, and treatment or programs for sex offenders or perpetrators of violence are noncovered. Facilities that do not participate with Blue Cross/BCN (for Michigan facilities) or with the member’s local Blue plan (for out-of-state facilities) are also not payable.

The policy also specifies that nonmedical services — including but not limited to enrichment programs, dance therapy, art therapy, music therapy, equine therapy, yoga and other movement therapies, 'ropes' courses, guided imagery, consciousness raising, socialization therapy, social outings, and educational or preparatory courses or classes — are not payable separately. Note that the policy allows these activities to be furnished as part of a treatment program, but they will not be reimbursed when billed separately.

Continued stay is payable only when supported by documentation demonstrating ongoing clinical need. The policy states that continued stay without documentation of a thorough evaluation, an individualized treatment plan, and convincing justification is not covered. Services or continued stay after a physician has discharged the member, or when participation is initiated or prolonged solely for socioeconomic, domestic, or housing reasons, are likewise noncovered.

Additional noncoverage triggers include services provided beyond the period necessary for care and treatment, services provided when the member fails to be engaged or is not participating after a reasonable period, and services primarily intended to maintain gains from another program. Admissions or services without prior authorization are not payable, and extra charges for therapy sessions longer than 90 minutes are not reimbursed.

Authorization, Documentation, and Billing Actions for Providers

    Prior Authorization

    Prior Authorization Required

Residential psychiatric treatment requires prior authorization and must meet medical necessity, use and quality criteria. Providers whose members' care is managed by Blue Cross Behavioral Health should refer to Blue Cross Behavioral Health: Frequently asked questions for providers for information about requesting prior authorization. Blue Cross Behavioral Health uses medical necessity criteria to make determinations on prior authorization requests; these criteria are available in the Behavioral Health Services section of the Services That Need Prior Authorization webpage at bcbsm.com. For Blue Cross commercial members whose behavioral health care is managed by an entity other than Blue Cross Behavioral Health, refer to the list of Utilization Management and Care Management Opt-outs.

Level of Care Definitions and Thresholds

Residential — Level of care criteria

Admission clinical rationale: Member no longer requires inpatient intensity but requires 24/7 supervision and cannot be safely treated in less restrictive settings; multidisciplinary treatment necessary.
Continued stay justification: Ongoing documentation of individualized treatment, progress toward goals, and need for 24/7 supervision; absence of documentation or clinical justification makes continued stay noncovered or not medically necessary.

    Permitted Treatment Modalities

Group and individual psychotherapy, multidisciplinary rehabilitation services

Group therapy requirements: Group therapy minimum 12.5 hours/week; sessions 60–90 minutes; at least two psychological process-type groups per week; remaining groups may be expressive or educational.12.5 hours/week

    Key Definitions

Psychiatric residential treatment facility — definition and licensing
DefinitionState-licensed facility providing 24-hour mental health care and treatment seven days a week.
Licensing examplesMay be licensed as a child-caring institution, an adult foster care facility, or another type of health care facility that meets Blue Cross/BCN requirements.
Scope of servicesProvides room and board, 24/7 supervision, nursing care, psychiatric services, individual and group therapy, and age-appropriate resources (e.g., educational program for children).

    Background and Scope

Residential psychiatric treatment is intended for members who require care more intensive than outpatient services but who do not require inpatient hospital-level intensity. It is delivered in a state-licensed facility providing 24-hour supervision staffed by a multidisciplinary treatment team and includes room, board, supervision, nursing care, psychiatric services, individual and group therapy, and, where applicable, educational services for children or age-appropriate resources for geriatrics.

The program’s purpose is stabilization and treatment with the goal of returning the member to a less restrictive setting. Operational and clinical requirements include timely psychiatric involvement and active treatment planning — for example, a face-to-face or audio-visual evaluation by the attending psychiatrist within 48 hours of admission, development of an initial plan of care within 72 hours, a psychiatrist onsite at least 2 days per week, and programmatic therapy expectations such as a minimum of 12.5 hours of group therapy per week and appropriate individual therapy frequency.

    Documentation Required

    Clinical Documentation Requirements

Document the clinical reasons when a member cannot participate in group activities and maintain individualized plans of care. A psychiatric residential treatment facility must: perform a face-to-face or audio-visual evaluation by the attending psychiatrist within 48 hours of admission; provide 24/7 supervision and nursing care (onsite or on-call within 60 minutes) and have a psychiatrist on call; have a psychiatrist onsite at least two days per week; develop an initial plan of care within 72 hours and a more robust plan by the end of week 2; update each member's plan of care at least weekly. Each weekly plan must include a weekly face-to-face/audio-visual meeting with the member and caretaker/guardian/family unless clinically inappropriate (document the reason), medication management by an appropriate clinician at least twice weekly until stable then at least once weekly, ongoing medical services for comorbid conditions, integrated multidisciplinary treatment with coordination to community resources, individual therapy twice weekly, group therapy totaling at least 12.5 hours per week (60–90 minute sessions, at least two psychological sessions; remaining sessions may be expressive), recreational activities including minimum four hours planned treatment/activities each weekend day, and specialized provisions: certified school program for children/adolescents and geriatric-focused social needs for older members.

Consequences of No Prior Authorization and Noncovered Services

If prior authorization is not obtained: a participating (in-network) facility that provides the care cannot bill the member for the cost of the admission and services; a nonparticipating (out-of-network) facility may require the member to pay. Admissions or services provided before the effective date of coverage, admissions or continued stay without prior authorization, or services provided when the member is not engaged after a reasonable period may be noncovered or denied. Examples of noncovered items include admissions without prior authorization, services by nonparticipating facilities (for Michigan facilities, those not participating with Blue Cross or BCN), marital counseling, diversional therapy, services not focused on improving functioning, services beyond the period necessary, extra charges for sessions over 90 minutes, wilderness programs, transitional living centers, private rooms or apartments, therapeutic boarding schools, milieu therapies, custody or custodial care, court-ordered services, and programs for sex offenders.

    • Participating facility without prior authorization: cannot bill member
    • Nonparticipating facility without prior authorization: may require member to pay
    • Admissions/services before coverage effective date are not payable
    • Admission or services without prior authorization may be noncovered/denied
    • Services by nonparticipating facilities (per location rules) are not payable
    • List of specific noncovered services (see policy)
    Recreational and specialty therapies:
Recreational activities and therapeutic modalities (e.g., expressive therapies, equestrian/ropes exercises) may be included but not billed to the member separately; clinical judgment and documentation required if member cannot tolerate designated group activity amounts.
Educational and geriatric resources: For children and adolescents, a certified school program must be provided. Geriatric facilities must include activities and resources to meet social needs of older members with chronic mental illness.
DocumentationClinical judgment should be used to determine tolerability of required therapy; reasons a member cannot participate should be documented in the chart.
