Coverage Criteria for PCI

Covered Indications

PCI is considered MEDICALLY NECESSARY for the following conditions:

Medically necessary indications: Acute coronary syndrome (eg acute myocardial infarction, unstable angina).

Each listed indication is sufficient as a covered reason for PCI per policy.

Medically necessary indications: History of significant obstructive atherosclerotic disease.

Each listed indication is sufficient as a covered reason for PCI per policy.

Medically necessary indications: Restenosis of a coronary artery previously treated with intracoronary stent or other revascularization procedure (such as a stent, balloon angioplasty, or bypass graft).

Each listed indication is sufficient as a covered reason for PCI per policy.

Medically necessary indications: Chronic angina.

Each listed indication is sufficient as a covered reason for PCI per policy.

Medically necessary indications: Silent ischemia.

Each listed indication is sufficient as a covered reason for PCI per policy.

Diagnostic Cardiac Catheterization

Diagnostic cardiac catheterization to assess lesion(s) prior to intervention:

Diagnostic cath timing: Diagnostic cardiac catheterization to assess the nature of the lesion(s) prior to the intervention is MEDICALLY NECESSARY. The diagnostic cardiac catheterization may be performed at any time prior to the PCI, including the same day as the PCI.

Same-day diagnostic cath and PCI is recommended when medically appropriate; reasons for delaying intervention (eg excessive dye load, transfer, further planning) should be documented.

Intravascular Imaging and Physiologic Measurement

Adjunctive intravascular procedures considered medically necessary when clinically indicated:

Intravascular imaging: Intracoronary ultrasound (IVUS) or optical coherence tomography (OCT) may be considered MEDICALLY NECESSARY to assess the extent of coronary stenosis if angiography is equivocal, or to assess patency and integrity of a coronary artery postintervention.

Corresponds to CPT 92978 (initial vessel) and 92979 (each additional vessel).

Physiologic assessment: Intravascular Doppler velocity and/or pressure-derived coronary flow reserve measurement may be considered MEDICALLY NECESSARY to assess the degree of stenosis within a vessel.

Corresponds to CPT 93571 (initial vessel) and 93572 (each additional vessel).

Not Covered/Investigational Uses

Investigational uses: PCI is considered INVESTIGATIONAL for all other indications not listed as medically necessary.

Separation of diagnostic cardiac catheterization and percutaneous coronary intervention (PCI) for the purpose of avoiding a multiple‑surgery pricing reduction or for the convenience of physician or hospital scheduling is considered inappropriate and may be subject to review and denial for medical necessity. The determination to stage procedures is a clinical decision reserved to the interventional cardiologist and should be based solely on the patient’s clinical needs (for example, cumulative dye load or the need to correlate angiographic findings with other test results).

A repeat diagnostic catheterization within three months of a prior diagnostic catheterization — performed before completion of a planned staged intervention — is generally considered NOT MEDICALLY NECESSARY unless there has been a new clinical event or a clear change in symptoms, physical examination, or diagnostic test results. When an intervention is deliberately delayed for clinical reasons (e.g., to limit contrast exposure or to await additional information), those reasons must be documented in the medical record.

Background

Percutaneous coronary intervention (PCI) is a nonsurgical, catheter‑based procedure performed to open stenotic coronary arteries. PCI therapies commonly include balloon angioplasty, atherectomy, thrombectomy, and placement of intracoronary stents to restore blood flow in obstructive coronary artery disease, and are used in clinical scenarios such as acute coronary syndromes (including acute myocardial infarction and unstable angina), chronic angina, restenosis after prior revascularization, and selected multivessel disease. Intracoronary imaging (e.g., IVUS/OCT) and physiologic measurements (e.g., Doppler/FFR) may be employed to assess lesion significance and postintervention results.
