    Coverage Criteria

inv-01: Coverage criteria — Covered when ALL of the following are met

Covered when ALL of the following are met

Periodontal maintenance coverage criteria: 1) History of periodontal therapy and continuous care not exceeding 12 months.

    From policy criteria item 1

    2) Prior history of definitive periodontal therapy with dates and specific treatment rendered.

Documentation of prior definitive therapy required (policy criteria item 2)

    3) Post-initial therapy evaluations and treatment planning recommendations following completion of periodontal maintenance are considered integral components of this procedure.

    Policy criteria item 4

    4) A periodontal diagnosis including staging and grading per current American Academy of Periodontology (AAP) and American Dental Association (ADA) guidelines is present.

    Policy criteria item 6

    5) Procedure is not appropriate when there is no history of periodontal disease requiring definitive periodontal treatment.

Policy criteria item 5 (exclusion) - must have prior disease and definitive treatment

A periodontal maintenance procedure is considered not appropriate when there is no history of periodontal disease requiring definitive periodontal treatment. This exclusion reflects the requirement that maintenance follows prior therapeutic care and is not intended as an initial preventive or routine prophylaxis for patients without a documented history of treated periodontal disease.

Use of periodontal maintenance for patients without prior definitive periodontal treatment or without a documented periodontal diagnosis (including staging and grading per current AAP and ADA guidelines) is considered not appropriate. Providers should ensure that the member has a prior history of definitive periodontal therapy with dates and specific treatments recorded before billing maintenance services.
