inv-01: Appropriate use criteria for LDCA/LDAA

Covered when ALL of the following are met

Prerequisite therapy: Definitive non-surgical and/or surgical periodontal treatment (including scaling and root planing) has been completed and allowed time to heal.6 weeks to 6 months post SRP or surgery

LDCA/LDAA are adjunctive after SRP/surgery

Target site characteristics: Site demonstrates active/refractory disease: periodontal pocket measuring 5 mm or greater with spontaneous bleeding, bleeding on probing, or suppuration, and history of increasing pocket depth with associated attachment loss.>= 5 mm

Isolated refractory sites at maintenance appointments are included

Site distribution: Localized or isolated refractory sites (appropriate as adjunctive or during maintenance); not intended for generalized periodontitis where more extensive or surgical therapies may be required.

Generalized periodontitis should prompt consideration of comprehensive/surgical therapy

Documentation: A current (within 12 months), dated, post-periodontal maintenance and/or post-periodontal surgery 6-point periodontal charting is required.within 12 months

Use in medically compromised patients requires additional physician documentation and case-by-case review

Treatment limitations: LDCA/LDAA are not appropriate for use to regenerate alveolar bone; such application is considered experimental and investigational and is excluded.

Only natural teeth with favorable periodontal outcomes will be considered for benefit

Adjunctive use after SRP — timing and clinical context

LDCA/LDAA modalities are adjunctive therapies and are considered only after definitive non-surgical therapy (scaling and root planing) has been performed and allowed time to heal. Allow adequate healing time before adjunctive local delivery: typically 6 weeks to 6 months after scaling and root planing or periodontal surgery.

Adjunctive timing: perform 6 weeks to 6 months post scaling and root planing or periodontal surgery
LDCA/LDAA are for pockets ≥5 mm with signs of active/refractory disease (spontaneous bleeding, bleeding on probing, suppuration, or documented increasing pocket depth/attachment loss)
If multiple recurrent/residual inflammatory sites persist after SRP, consider more extensive periodontal modalities (including surgical options) rather than repeat adjunctive LDCA/LDAA

Documentation Required

Required documentation — 6-point periodontal charting

Documented periodontal assessment is required for review and payment. Maintain current periodontal charting and supporting clinical documentation.

A current (within 12 months), dated 6-point periodontal charting is required (post periodontal maintenance and/or post-periodontal surgery as applicable)
Include documentation of treated sites, pocket depths, bleeding on probing, suppuration, and rationale for local antimicrobial delivery

Medically compromised patients — case-by-case review

Use of LDCA/LDAA in medically compromised patients will be reviewed on a case-by-case basis and requires supporting physician documentation.

Submit physician documentation describing the medical condition and justification for local antimicrobial therapy
Coverage for medically compromised patients is not automatic; benefits will be determined after clinical review

Denial Risk

Bundled denial risk — related services may be denied

When services are performed in conjunction with or in preparation for a non-covered or denied service, related services may also be denied. Verify coverage for all associated procedures before billing.

If SRP or another foundational therapy is non-covered or denied, adjunctive LDCA/LDAA services tied to that therapy may also be denied
