OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Medically Necessary Criteria

Covered when ALL of the following are met

Primary coverage criteria: 1) Presence of mild to moderate periodontal disease requiring elimination of suprabony pockets or gingival recontouring for supra-bony pockets with normal bony configuration; 2) Failure of nonsurgical periodontal therapy (e.g., scaling and root planing) to remove subgingival deposits; 3) Current (within 12 months) dated 6‑point periodontal charting showing pocket depths of at least 5 mm; 4) Current (within 12 months) diagnostic-quality pretreatment radiographs showing the periapical area and alveolar crest; 5) Completion of initial periodontal therapy with at least a four-week healing interval prior to surgical treatment; 6) Benefits limited to two quadrants per date of service (exceptions may be allowed on a case‑by‑case basis) and generally limited to one periodontal surgical procedure in a [36/60] month period per single tooth or multiple teeth in the same quadrant (group contract dependent).pocket depth >=5 mm

Exceptions may be allowed on a case-by-case basis

Coverage for gingivectomy or gingivoplasty is limited to procedures addressing supra‑bony periodontal pockets and recontouring of gingiva where the bony architecture is normal. To be considered medically necessary, ALL of the following must be documented: completion of initial nonsurgical periodontal therapy (e.g., scaling and root planing) with at least a four‑week healing interval; a current (within 12 months) dated 6‑point periodontal charting showing pocket depths of at least 5 mm; and current (within 12 months) diagnostic‑quality pretreatment radiographs demonstrating the periapical area and alveolar crest. Benefits are generally limited to two quadrants per date of service, and group contracts commonly limit coverage to one periodontal surgical procedure per [36/60] month period per tooth or multiple teeth in the same quadrant. Exceptions may be allowed on a case‑by‑case basis.

The procedure is not indicated for infrabony defects or when pockets extend below the mucogingival junction, when access to alveolar bone is required, or when there is only minimal attached keratinized tissue. Gingivectomy to provide access for restorative work may be group contract dependent but is typically considered inclusive when clinically indicated.

Gingivectomy performed solely for esthetic reasons is not covered in certain situations. Specifically, gingivectomy performed within six months of orthodontic treatment is considered cosmetic and is not benefited. Gingivectomy performed for the purpose of correcting altered passive eruption is not benefited.

Additionally, procedures that are incidental to or inclusive with restorative work should be evaluated per applicable code guidance (for example, D4212 considerations) and group contract terms. Providers should ensure documentation supports medical necessity; lack of required documentation or failure to meet pocket depth or nonsurgical therapy requirements may result in denial.

    Provider Actions & Documentation Requirements

    Prior Authorization

    Required prior nonsurgical therapy

Completion of initial nonsurgical periodontal therapy is required prior to consideration of gingivectomy/gingivoplasty. Allow a minimum of four weeks after scaling and root planing (SRP) or other initial periodontal therapy for tissue healing and reassessment to determine success of nonsurgical treatment. Exceptions may be allowed on a case-by-case basis when documented clinical justification is provided.

    • Required: completion of scaling and root planing or equivalent initial periodontal therapy prior to surgical intervention.
    • Minimum healing interval: at least 4 weeks between completion of nonsurgical therapy and any periodontal surgery.
    • Documentation: clinical notes describing response to nonsurgical therapy and rationale for proceeding to surgery if performed earlier.
    Documentation Required

    Prior authorization / claim documentation

Submit current (within 12 months) diagnostic documentation with prior authorization and/or claims to support medical necessity for periodontal surgery. Documentation must include a full 6‑point periodontal chart and diagnostic-quality radiographs.

    Background

A gingivectomy is the surgical removal of diseased gingival tissue to eliminate supra‑bony periodontal pockets and reduce pocket depth in the presence of periodontal disease. It is intended to remove inflamed or overgrown tissue and facilitate periodontal health.

A gingivoplasty is reshaping of otherwise healthy gingival tissues in the absence of periodontal pockets. The goal is to create a physiologic gingival contour and proper gingival form rather than to remove diseased tissue.

    Definitions

inv-11: Gingivectomy — surgical removal of diseased gum tissue
DefinitionSurgical removal of diseased gum tissue in the presence of periodontal disease.
IndicationTreatment of supra‑bony periodontal pockets to reduce pocket depth and allow periodontal healing after nonsurgical therapy has failed.
Access for restorative proceduresMay be performed to allow access for a restorative procedure (group contract dependent for inclusion).
ContraindicationsNot indicated for infra‑bony pockets, pockets extending below the mucogingival junction, minimal attached keratinized tissue, or procedures requiring access to alveolar bone.
Timing/limitationsConsidered cosmetic when performed within six months of orthodontic treatment; benefits generally limited to one periodontal surgical procedure per [36/60] months per tooth or quadrant (group dependent).

    • 6‑point periodontal charting: dated and current within 12 months; must show pocket depth recordings with minimum 5 mm pockets in the area proposed for surgery.
    • Post-therapy documentation: charting after completion of nonsurgical therapy and/or periodontal maintenance when applicable (current within 12 months).
    • Radiographs: diagnostic quality pretreatment radiographs demonstrating periapical area and alveolar crest (current within 12 months).
    • Limit: benefits generally limited to two quadrants per date of service; group contract exceptions may apply.
    • Frequency: typically limited to one periodontal surgical procedure per 36 or 60 months (group-dependent) — verify group contract.

Additional coverage and coding notes to guide benefit determination and billing.

    • Gingivectomy to allow access for crown(s)/restoration(s) is group-specific and often considered inclusive to the primary restorative procedure; submit supporting restorative documentation.
    • Use of lasers/electrosurgery is considered an adjunct and is not eligible for a separate additional benefit.
    • Gingivectomy for infra-bony pockets or pockets extending below the mucogingival junction is contraindicated and not covered.
    • Gingivectomy for removal of inflamed/hypertrophied tissue around partially erupted or impacted teeth should be coded as D7971 (pericoronal tissue excision).
    • Gingivectomy performed within six months of orthodontic treatment is considered cosmetic and not covered.
    • Ensure periodontal diagnosis follows current AAP/ADA guidelines including staging and grading where applicable.
inv-12: Gingivoplasty — reshaping of healthy gum tissue
DefinitionReshaping of otherwise healthy gum tissues in the absence of periodontal pockets.
Use casePerformed to improve gingival contours or esthetics and may be used to allow access for restorative procedures when pockets are not present.
Not for infra‑bony diseaseNot indicated for treatment of infra‑bony pockets or pockets extending below the mucogingival junction.
Adjunct techniquesUse of lasers/electrosurgery is considered adjunctive and not eligible for additional benefit beyond primary procedure.
Cosmetic considerationProcedures to correct altered passive eruption are not benefited; gingivoplasty performed within six months of orthodontic treatment may be considered cosmetic depending on circumstance.
