Coverage Criteria for Gingivectomy / Gingivoplasty

    Coverage criteria

Covered when ALL of the following are met

Primary coverage criteria: 1) Presence of mild to moderate periodontal disease with suprabony pockets or gingival enlargement/asymmetry requiring recontouring (suprabony pockets with normal bony configuration).

    See required documentation below.

Pocket depth requirement: 2) Current (within 12 months) 6‑point periodontal charting documenting pocket depths of at least 5 mm after completion of nonsurgical periodontal therapy/maintenance.>= 5 mm

Charting must be dated within 12 months and obtained after nonsurgical therapy; minimum 5 mm pocket depth required.

Radiographic documentation: 3) Current (within 12 months) diagnostic quality pretreatment radiographs demonstrating the periapical area and alveolar crest.

Radiographs must be diagnostic quality and dated within 12 months.

Nonsurgical therapy prerequisite: 4) Completion and documentation of initial nonsurgical periodontal therapy (e.g., scaling and root planing) with at least four weeks of healing prior to surgery; nonsurgical therapy should be attempted and documented as unsuccessful for persistent subgingival deposits.

Allow minimum four weeks healing to assess success of non-surgical therapy.

Benefit limits and code applicability: 5) Procedure is limited to two quadrants per date of service and is generally limited to one periodontal surgical procedure per 36/60 month period per single tooth or multiple teeth in the same quadrant, subject to group contract terms; gingivectomy to allow access for restorative procedures may be considered inclusive per group contract rules.

Exceptions may be allowed case-by-case and benefits are group contract dependent.

Adjunct techniques: 6) Use of lasers/electrosurgery is considered an adjunct and is not eligible for an additional separate benefit.

    Laser/electrosurgery considered adjunctive treatment.

Gingivectomy/gingivoplasty is contraindicated for treatment of infra-bony pockets and for pockets that extend below the mucogingival junction. The policy also excludes procedures when there is only a minimal amount of attached keratinized tissue or when the procedure would require access to the alveolar bone (these situations are not appropriate for gingivectomy/gingivoplasty under this benefit).

Gingivectomy performed solely to correct altered passive eruption is not a covered benefit. Procedures that are incidental to the placement of crown(s) or restorations — including those considered inclusive when done with restorative work per group contract rules — may be treated as incidental or not separately benefited if diagnostic information does not support medical necessity. Gingivectomy done to provide access for a restorative procedure (D4212) is group contract dependent and may be considered inclusive; when diagnostics show adequate periodontal support, the access procedure may not be necessary. Gingivectomy performed for cosmetic purposes (for example, within six months of orthodontic treatment) is also considered cosmetic and not covered as a separate benefit.

Contraindications and Incidental / Cosmetic Procedures

Contraindications and incidental/cosmetic procedure guidance for Gingivectomy/Gingivoplasty. Procedures are contraindicated for infra-bony pockets, pockets extending below the mucogingival junction, where minimal attached keratinized tissue exists, or when access to alveolar bone is required. Gingivectomy to remove inflamed/hypertrophied tissue around partially erupted or impacted teeth should be coded as D7971. Gingivectomy for altered passive eruption is not covered. When performed to allow access for crown(s)/restoration(s), gingivectomy (D4212) is often considered inclusive to the restorative procedure and may be incidental; evaluate per D4211/D4212 guidance and group contract. If diagnostics indicate sufficient periodontal support to allow restorative access without gingival surgery, the procedure may not be necessary and may be considered incidental. Gingivectomy is considered cosmetic when performed within six months of orthodontic treatment. Use of lasers/electrosurgery is considered an adjunct and is not eligible for an additional benefit.

    • Contraindications: infra-bony pockets; pockets extending below the mucogingival junction; minimal attached keratinized tissue; procedures requiring access to alveolar bone.
    • Gingivectomy for access to restorations (D4212) may be considered inclusive or incidental depending on documentation and group contract; evaluate per D4211/D4212 guidance.
    • Gingivectomy for pericoronal tissue around partially erupted/impacted teeth: use D7971.
    • Gingivectomy for altered passive eruption: not benefited.

    Background

Gingivectomy is the surgical removal of diseased gingival tissue to eliminate suprabony pockets and restore normal gingival architecture, typically indicated when nonsurgical therapy has failed or when suprabony pockets with normal bony configuration exist. Gingivoplasty is the reshaping of gingival tissue — often of otherwise healthy gingiva — to recontour the gingiva for functional or aesthetic reasons, including correction of drug‑induced gingival overgrowth or to facilitate restorative access when appropriate.

    Key Definitions

    Gingivectomy: definition
DefinitionGingivectomy: surgical removal of diseased gum tissue in the presence of periodontal disease
IndicationUsed to eliminate suprabony pockets and restore normal gingival architecture when nonsurgical therapy is insufficient
Relation to restorative careMay be performed to allow access for a restorative procedure on a tooth
    Gingivoplasty: definition
DefinitionGingivoplasty: reshaping of otherwise healthy gum tissues in the absence of periodontal pockets
IndicationPerformed to reshape gingiva for restorative access, cosmetic contouring, or to correct drug‑induced gingival enlargement when pockets are not present

  1. Gingivectomy within 6 months of orthodontic treatment: considered cosmetic (not covered).
  2. Lasers/electrosurgery: adjunctive only; no additional benefit.
  3. Documentation Required

    Required Documentation

Required documentation to support medical necessity for gingivectomy/gingivoplasty. Provide current, dated diagnostic records completed within 12 months of the proposed surgical date and retain in the patient record.

    • Current (within 12 months) dated 6‑point periodontal charting with pocket depth recordings showing minimum 5 mm pockets where surgery is indicated.
    • If performed after nonsurgical therapy, provide a current (within 12 months) dated 6‑point periodontal charting completed after completion of nonsurgical periodontal therapy and/or periodontal maintenance.
    • Current (within 12 months) diagnostic-quality pretreatment radiographs showing the periapical area and the alveolar crest.
    • Documentation of completion of initial periodontal therapy (scaling and root planing) with at least a four‑week healing interval prior to any surgical treatment, unless an exception is documented and approved on a case‑by‑case basis.
    • Record retention: include dated treatment notes describing prior nonsurgical therapy, response to therapy, clinical findings, and the rationale for surgical intervention.
    Prior Authorization

    Non-surgical Therapy Prerequisite

Non-surgical therapy prerequisite: document attempts and outcomes of scaling and root planing prior to approving gingivectomy/gingivoplasty. Allow adequate healing time to assess response to nonsurgical care.

    • Completion of initial periodontal therapy (e.g., scaling and root planing) is required prior to surgical intervention, with a minimum four‑week interval to allow tissue healing and reassessment.
    • Evidence of inadequate response to nonsurgical therapy must be documented (clinical findings, residual pocket depths >=5 mm, continued inflammation, persistent subgingival deposits).
    • Exceptions to the prerequisite may be considered on a case‑by‑case basis and must be supported by clinical documentation explaining the rationale for immediate surgical treatment.
Distinction from gingivectomyUnlike gingivectomy, gingivoplasty is not intended to treat periodontal pockets that extend below the gingival margin
