OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    inv-01: COVERAGE CRITERIA

Covered when ALL of the following are met:

    Primary coverage criteria

    • Required documentation and findings: Dated post-initial therapy 6-point periodontal charting within 12 months showing pocket depth recordings of a minimum of 5 mm AND dated pretreatment radiographs within 12 months showing periapical area and an undistorted image of the alveolar crest.
    • Anatomic indication: Presence of supra-bony pocket depths where increased access to root surfaces is needed; procedure is for periodontal defects involving natural teeth only (not implants).
    • Procedure scope limits: Limited to two quadrants per date of service (exceptions case-by-case); osseous recontouring is not accomplished with this procedure; use of lasers is adjunctive and not separately reimbursable.
    • Additional acceptable indications: Gingival flap procedure for exploratory purposes to determine presence of a cracked tooth or fractured root may be beneficial; procedure may be performed in the presence of minimal amounts of attached keratinized tissue.
Diagnosis standard: Periodontal diagnosis must include staging and grading per current American Academy of Periodontology (AAP) and American Dental Association (ADA) guidelines.

Gingival flap procedures will not be considered when performed around implants. Documentation that the periodontal defect involves natural teeth only is required; procedures involving implant sites may be denied.

Gingival flap procedures and apically positioned flaps are not performed for treatment of pockets extending below the mucogingival junction. Providers must confirm pocket location relative to the mucogingival junction in submitted charting and operative notes.

The use of lasers is considered an adjunct to gingival flap procedures and is not eligible for an additional or separate benefit. Any laser application should be documented as part of the primary procedure rather than billed separately.

    Provider Actions & Documentation Requirements

    Documentation Required

    Provider Documentation & Procedural Limits

Gingival flap procedures and apically positioned flap (CDT D4240, D4241, D4245) require documentation that nonsurgical therapy (such as scaling and root planing) was attempted and unsuccessful prior to performing the procedure. Provide dated clinical records demonstrating failure of conservative therapy and that a soft tissue flap was reflected or planned to allow debridement and removal of granulation tissue. Procedures performed around implants are not considered and may be denied.

    • Prior nonsurgical therapy documented (e.g., scaling and root planing) with evidence of failure
    • Current (within 12 months) dated 6‑point periodontal charting after nonsurgical therapy or maintenance showing minimum 5mm pocket depths
    • Current (within 12 months) dated pretreatment radiographs with undistorted view of the alveolar crest
    • Chart notes demonstrating that a soft tissue flap was reflected/resected or planned to allow debridement and removal of granulation tissue
    • Procedures performed around implants are not eligible and may be denied

    Background

Gingival flap procedures involve reflecting a soft tissue flap to allow debridement of the root surface and removal of granulation tissue. An apically positioned flap is a surgical flap used to maintain or increase the zone of keratinized tissue and to eliminate supra-bony pockets without osseous manipulation. These procedures are indicated when nonsurgical therapy (scaling and root planing) has not adequately removed subgingival deposits and greater access to root surfaces is needed.

    Definitions

    Gingival flap procedure
DefinitionA reflected soft tissue flap created to allow debridement of the root surface and removal of granulation tissue.
PurposeProvides increased access to root surfaces for thorough debridement.
Operative limitDoes not include osseous recontouring or manipulation of the osseous bone of the periodontium.
    Apically positioned flap
DefinitionA surgical flap positioned apically to maintain an adequate zone of keratinized tissue to achieve pocket elimination.
Clinical intentUsed to eliminate supra-bony pockets without osseous manipulation.
