General clinical criteria for coverage

Covered when ALL of the following are met

Radiographic indication: A diagnostic radiograph documents less than three millimeters of sound natural tooth structure between the restorative margin and the alveolar crest.< 3 mm

When indications are not evident by radiographic examination, additional patient records may be requested.

Periodontal health: Procedure is performed only in a periodontally healthy environment (no active periodontal disease).

Clinical crown lengthening will not be considered when performed in conjunction with any procedure that addresses periodontal treatment for unhealthy periodontal tissues within the same quadrant on the same date of service; when performed in conjunction with osseous surgery for periodontal disease the crown lengthening procedure is inclusive with osseous surgery.

Tooth type: Only natural teeth are eligible for clinical crown lengthening.

Clinical crown lengthening will be considered for natural teeth only.

Indication for restoration: There must be inadequate tooth structure exposed to retain a dental restoration (for example, subgingival caries or fracture) requiring removal of soft and hard tissue to enable restoration.

Clinical crown lengthening will be considered only when subgingival caries or fracture requires removal of soft and hard tissue to enable restoration; it will not be considered for structural loss due to wear, erosion, attrition, abrasion, or abfraction.

Cosmetic and elective exclusions: Procedure performed for cosmetic reasons or to correct congenital/developmental defects is considered elective and is not allowed.

'Troughing' of soft tissue as part of crown preparation or to visualize margins does not meet the D4249 descriptor.

Crown timing and healing: A minimum of 6 weeks must be allowed for healing of bone and soft tissue following clinical crown lengthening prior to final restoration.6 weeks

If crown lengthening is performed on the same date of service as the crown, D4249 is considered inclusive with the crown.

Crown-to-root ratio and prognosis: If resulting bone removal creates an inadequate crown-to-root ratio compromising long-term prognosis, the procedure is not appropriate.

Removal of hard and soft tissue that compromises long‑term progress of the remaining tooth indicates no benefit.

Clinical crown lengthening will not be considered when performed in conjunction with any procedure that addresses periodontal treatment for unhealthy periodontal tissues within the same quadrant on the same date of service. Examples include, but are not limited to, gingivectomy, frenectomy, distal wedge reduction, grafting, and scaling and root planing; these procedures are considered an integral component of periodontal treatment and are not separately allowed when done with crown lengthening. Additionally, when crown lengthening is performed in conjunction with osseous surgery for periodontal disease, the crown lengthening is considered inclusive with the osseous surgery and not separately considered for coverage.
