OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria

    Initial criteria for coverage

Covered when ALL of the following are met

Documentation prerequisites: Current (within 12 months) dated pretreatment periodontal chart documenting millimeters of recession (CEJ to gingival margin), millimeters of attached gingiva remaining, and 6‑point pocket depths; history of progressive recession within 12 months; notation of high frenum attachments; number of teeth affected; photographic documentation of areas demonstrating recession as required; current (within 12 months) pretreatment radiographs as required; indication of root sensitivity; relationship to cervical caries/existing restorations; and a narrative stating the rationale for the procedure.
Tissue measurements or extraordinary circumstances: Either periodontal charting indicates a minimum of 2 millimeters of keratinized gingival tissue recession AND 1 millimeter or less of attached gingiva remaining, OR extraordinary circumstances (e.g., frenum involvement, chronic inflammation) justify intervention.>=2 mm recession AND <=1 mm attached gingiva

    Extraordinary circumstances may permit exceptions.

Clinical appropriateness: Procedure is not performed solely for cosmetic reasons; crown:root ratio poorer than 1:1 may lead to denial; provide current AAP/ADA periodontal diagnosis including staging and grading.

    Procedure limits and inclusions

Benefit limits and inclusions: Benefits are group contract dependent but generally limited to one periodontal surgical procedure in a [36/60] month period per single tooth or multiple teeth in the same quadrant; evaluation and post‑operative care are included for three months and any surgical re‑entry for three years (contract dependent); frenectomy/frenuloplasty is inclusive when performed in the same area and on the same date as a soft tissue graft; biological dermal matrix materials are not benefitted when submitted in conjunction with soft tissue grafting.

    Implant-specific and adjunct assessments

Implant considerations: If implant(s) are approved as necessary and appropriate, additional mucogingival procedures should be assessed based on submitted diagnostics, narrative/rationale, and photographs to determine necessity and appropriateness.

Mucogingival surgery and soft tissue grafting performed solely for cosmetic purposes are not benefitted. Additionally, biological dermal matrix materials are not benefitted when submitted in conjunction with soft tissue grafting.

Services performed in conjunction with or in preparation for a non‑covered or denied service will also be not covered or denied.

When a member demonstrates good oral health with maintained plaque control (no plaque buildup), mucogingival surgery and soft tissue grafting are considered inappropriate and may be not medically necessary.

Clinical documentation should confirm plaque control and oral hygiene status prior to approving mucogingival procedures, since maintained plaque control is a key factor in determining the need for surgical intervention.

    Coding

    CDT procedure codes (examples listed)CDT
D4273Pedicle soft tissue graft procedure = Autogenous connective tissue graft procedure (including donor and recipient surgical sites) - 1st tooth, implant or edentulous tooth position in graft
D4283Pedicle soft tissue graft procedure = Autogenous connective tissue graft procedure (including donor and recipient surgical sites) - each additional contiguous tooth, implant or edentulous tooth position in same graft site
D4274Pedicle soft tissue graft procedure = Mesial/distal wedge procedure, single tooth
D4275Pedicle soft tissue graft procedure = Non - autogenous connective tissue graft procedure (including recipient site and donor material) first tooth, implant or edentulous tooth position in graft
D4285Pedicle soft tissue graft procedure = Non-autogenous connective tissue graft procedure (including recipient surgical site and donor material) - each additional contiguous tooth, implant or edentulous tooth position in same graft site
D4276Pedicle soft tissue graft procedure = Combined connective tissue and pedicle graft, per tooth
D4277Pedicle soft tissue graft procedure = Free soft tissue graft procedure (including recipient and donor surgical sites) first tooth, implant or edentulous tooth position in graft
D4278Pedicle soft tissue graft procedure = Free soft tissue graft procedure (including recipient and donor surgical sites) each additional contiguous tooth, implant or edentulous tooth position in same graft site
D7340Vestibuloplasty - ridge extension (secondary epithelialization)
D7350Vestibuloplasty - ridge extension (including soft tissue grafts, muscle reattachment, revision of soft tissue attachment and management of hypertrophied and hyperplastic tissue)
Keratinized gingival tissue recession and attached gingiva — thresholds and exception note
Minimum recession requiredKeratinized gingival tissue recession >= 2 mm is required (except in extraordinary circumstances).
Maximum attached gingiva remainingAttached gingiva remaining must be <= 1 mm to meet criteria.
Extraordinary circumstances exceptionExceptions may be made for extraordinary circumstances (e.g., frenum involvement, chronic inflammation) that justify intervention despite thresholds.
Documentation time windowMeasurements must be from a current (within 12 months), dated pretreatment periodontal chart documenting millimeters of recession and attached gingiva remaining.

    Provider Actions and Documentation

    Prior Authorization

Prior authorization and documentation required (contract dependent)

Authorization/benefit determination is contract dependent; submit supporting diagnostics, narrative/rationale, and photographs when requesting prior authorization or benefits. CDT procedure codes listed in the policy are used for reporting and adjudication but inclusion does not guarantee coverage.

    • Submit pretreatment diagnostics (periodontal charting, radiographs) and photographic documentation as required.
    • Include a narrative stating the clinical rationale for the procedure.
    Note

Adhere to policy documentation and contract-dependent limits

Follow the policy's documentation and coverage criteria when submitting claims and prior authorization requests; ensure submitted materials match the specific requirements listed in the policy.

    • Provide all items listed in the pretreatment documentation requirements (periodontal charting, history of progressive recession, photos, radiographs, narrative).

    Background

Mucogingival surgery and soft tissue grafting are periodontal surgical procedures intended to correct anatomic and functional soft tissue problems around teeth and implants. Typical indications include progressive gingival recession with loss of attached gingiva and root exposure, absence or reduced width of keratinized attached gingiva, high frenum attachments, inadequate vestibular depth, and periodontal pocketing that extends beyond the mucogingival junction.

These procedures are used to improve periodontal health and function by increasing tissue thickness or width, covering exposed root surfaces to reduce sensitivity, preventing further recession, deepening the vestibule, or eliminating frenal pulls. Grafts may be autogenous (e.g., palatal donor connective tissue) or non-autogenous (e.g., tissue‑bank/dermal matrix), and specific graft types include connective tissue, free gingival, pedicle, and combined connective tissue and pedicle grafts.

Clinical factors that influence the need for treatment include chronic marginal inflammation, root sensitivity, crown‑to‑root ratio, and the documented progression of recession. Documentation of these findings is required to demonstrate medical necessity.

    Definitions

Types of grafts — connective tissue, free gingival, pedicle, combined connective tissue and pedicle
Connective tissue graftAutogenous connective tissue grafts (typically palatal donor) used to thicken tissue and cover roots.
Free gingival (free soft tissue) graftFree soft tissue grafts (including recipient and donor surgical sites) for augmenting attached gingiva.
Pedicle graftPedicle soft tissue graft procedures (tissue repositioned from adjacent area) to cover recession.
Combined connective tissue and pedicle graftProcedures combining connective tissue graft with a pedicle graft to address complex defects.
Purpose of graftsGrafts are used to thicken tissue, cover exposed roots, reduce sensitivity, and prevent further recession.

  1. Recognize that benefits and limits are group contract dependent (e.g., number of procedures per 36/60 months).
  2. Documentation Required

Required pretreatment documentation (within 12 months)

Prior to mucogingival surgery, document a current (within 12 months) dated pretreatment periodontal chart including millimeters of recession (CEJ to gingival margin), millimeters of attached gingiva remaining, and 6‑point pocket depths, and include photographic documentation of the areas demonstrating recession.

    • Document history of progressive recession within 12 months prior to treatment.
    • Include notation of high frenum attachments, number of teeth affected, indication of root sensitivity, relationship to cervical caries/restorations, and a narrative rationale.
    • Provide current (within 12 months) pretreatment radiographs if required by prior dental history.
    Denial Risk

Denial risk: linked or cosmetic procedures will be denied/not covered

Services performed in conjunction with or in preparation for a non-covered or denied service will also be not covered or denied; procedures performed solely for cosmetic purposes are not benefitted.

    • Frenectomy/frenuloplasty is considered inclusive when performed in the same area and on the same date as a soft tissue graft (when covered).
    • Biological dermal matrix materials are not benefitted when submitted with soft tissue grafting.
