Concomitant therapy restriction: Requested drug is NOT given concomitantly with other biologic DMARDs or specified agents (eg, adalimumab, etanercept, apremilast, tofacitinib)

Arthritis Indications — specific arthritis indication criteria grouped

Covered when ALL of the following are met for the specific arthritis indications:

Rheumatoid Arthritis Initial Criteria: For rheumatoid arthritis, patient meets policy selection criteria (eg, age, diagnosis, prior DMARD failure) and for continuation must have experienced beneficial clinical response (eg, decreased joint pain, morning stiffness, improved function, decreased soft tissue swelling)
Polyarticular JIA Criteria: For polyarticular juvenile idiopathic arthritis, patient meets selection criteria including failure of one or more traditional DMARDs; continuation requires demonstrated clinical benefit (eg, improved range of motion, reduced pain, decreased morning stiffness, improved function, decreased corticosteroid dose, improved inflammatory markers); AND weight‑based dosing applied
Systemic JIA Criteria: For systemic juvenile idiopathic arthritis, patient had inadequate response to NSAIDs or corticosteroids prior to tocilizumab; continuation requires demonstrated clinical benefit (eg, resolution of fever, reduced rash, normalization of inflammatory markers, decreased corticosteroid needs, reduced joint symptoms)

Giant Cell Arteritis — covered when ALL of the following are met (alternate chunk set)

Covered when ALL of the following are met:

Diagnosis and prior therapy: Patient has giant cell arteritis and has failed treatment with systemic corticosteroid therapy unless contraindicated or there is clinical evidence steroids will be ineffective or cause harm
Dosing and combination: Dosing consistent with regimen (eg, IV 6 mg/kg every 4 weeks OR SC 162 mg weekly in combination with a steroid taper); AND continuation requires documented beneficial clinical response (eg, decreased headache, scalp or jaw pain, reduced fatigue, improved vision, reduced corticosteroid dosage, or improved inflammatory markers)
Diagnostic considerations: Temporal artery biopsy is diagnostic gold standard but clinical suspicion may warrant initiation of steroid therapy prior to confirmation

Covered Indications and Requirements — consistency with FDA indications and policy evidence

Covered when consistent with FDA-approved indications and supported evidence described in the policy

FDA Indication Alignment: Treatment is for an FDA‑approved indication for intravenous tocilizumab (rheumatoid arthritis, systemic juvenile idiopathic arthritis, polyarticular juvenile idiopathic arthritis, giant cell arteritis, cytokine release syndrome, systemic sclerosis‑associated interstitial lung disease, and hospitalized COVID‑19 where approved)
Efficacy Evidence: Clinical trials or pooled analyses described in the policy support efficacy for the indication (eg, ACR response rates for RA; JIA ACR30 and other trial endpoints for systemic and polyarticular JIA; pooled CAR‑T data for CRS)
Prior Authorization: Prior authorization is required for all codes listed on the Medical Policy

Investigational or experimental services, including drugs provided under the investigational arm of a clinical trial or for indications not addressed by the FDA, are not covered by the Health Plan unless coverage is specifically mandated by CMS. Coverage for routine services in certain IDE studies is limited as described in the policy. Use of tocilizumab products when patient selection criteria in this policy are not met may be considered investigational (see specific exclusions below).

Requests may be denied if the member’s benefits do not include coverage for investigational treatments or if medical necessity requirements in this policy are not satisfied.

Use of tocilizumab in indications or dosage forms that are not FDA‑approved is considered investigational and is not covered. Examples explicitly noted in the policy include intravenous use for systemic sclerosis‑associated interstitial lung disease and subcutaneous use for COVID‑19, as well as outpatient use of intravenous tocilizumab for COVID‑19.

Providers should confirm the formulation and indication requested are among those supported by the policy and FDA approvals; requests for non‑approved forms or indications should be expected to be denied as investigational.

Although multiple intravenous tocilizumab biosimilars are listed with indications largely aligned to Actemra, the policy notes that Tofidence does not carry the cytokine release syndrome (CRS) or systemic sclerosis‑associated interstitial lung disease (SSc‑ILD) indications. Providers must verify the biosimilar product requested supports the specific indication before requesting authorization.

Dosing that exceeds the policy’s specified limits for each indication is not covered. Examples of the stated thresholds include ≤8 mg/kg every 4 weeks for adult rheumatoid arthritis, ≤12 mg/kg every 2 weeks for systemic JIA patients <30 kg, ≤10 mg/kg every 4 weeks for polyarticular JIA patients <30 kg, and a maximum of 800 mg per infusion for cytokine release syndrome with no more than four total infusions per CRS event.

Prior authorization will require documentation that ordered doses are within these maximums; requests that propose doses beyond these limits should be expected to be denied as not meeting policy dosing criteria.

Use of tocilizumab is considered not medically necessary when required disease‑specific patient selection criteria or continuation criteria listed in the policy are not met. For continuation requests, the plan requires documentation of prior authorization (from this plan or a previous plan) and evidence of a beneficial clinical response from baseline as specified for the indication.

Examples of unmet criteria that would render use not medically necessary include failure to demonstrate prior trial of indicated conventional therapies (where required), lack of documented clinical improvement during prior therapy, concomitant use with prohibited biologic DMARDs, or dosing outside the policy limits.

For the purposes of this policy, investigational therapies are those not supported by sufficient credible evidence or not FDA‑approved for the requested indication or dosage form; investigational uses are not covered unless CMS requirements mandate coverage. Medically necessary services are those that meet nationally accepted standards of medical practice, are clinically appropriate in type, frequency, extent and duration, and are not primarily for convenience or more costly than an alternative of equivalent effectiveness.

Decisions about investigational versus medically necessary status will be based on FDA approval status, the policy’s clinical selection and continuation criteria, and the presence of peer‑reviewed evidence or specialty society guidance as described in the policy’s coding and clinical rationale sections.

Provider Requirements, Prior Authorization, and Documentation

    Prior Authorization

Prior authorization required — initial and continuation; include documentation

Obtain prior authorization for both initial and continuation IV tocilizumab therapy. For continuation requests, provide evidence that the patient received initial authorization from this plan OR documentation of authorization for an active course of treatment from a previous health plan, plus evidence of clinical benefit from baseline.

    • Initial and continuation therapy require prior authorization.
    • Continuation requires documentation of prior plan authorization OR initial authorization from this plan.
    • Include evidence of clinical benefit from baseline (indication-specific outcomes).
    Prior Authorization

Continuation authorization requires prior plan authorization or documentation of active course

For continuation, submit either the plan's prior authorization for the initial course OR documentation from the previous health plan showing authorization for an active course of treatment; include clinical response data demonstrating benefit.

    Background and Scope

Tocilizumab intravenous products (Actemra and its listed biosimilars) are targeted interleukin‑6 receptor antagonists used to treat multiple inflammatory conditions. The FDA‑approved intravenous indications referenced in the policy include rheumatoid arthritis, systemic juvenile idiopathic arthritis, polyarticular juvenile idiopathic arthritis, giant cell arteritis, cytokine release syndrome, systemic sclerosis‑associated interstitial lung disease, and hospitalized COVID‑19 where indicated. The policy also notes the approval history and that biosimilars (for example, Tofidence, Tyenne, Avtozma) have been approved with varying indication coverage compared to Actemra.

The policy specifies age thresholds, prior therapy requirements (such as failure of traditional DMARDs or inadequate response to NSAIDs/corticosteroids where applicable), TB screening requirements prior to initiation, dosing limits by indication and weight, restrictions on concomitant biologic DMARD use, and authorization/documentation expectations for continuation of therapy.
