OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Determinations for Soft Tissue Grafting

    Medical necessity criteria

Coverage determinations per policy statements A–C

A - Required condition: Soft tissue grafting is considered medically necessary when gingival recession with mucogingival involvement is present.

Policy A applies when mucogingival involvement is documented.

B - Permissible indications without mucogingival involvement

    • 1: Root abrasion, abfraction, or erosion
    • 2: Class V caries or defective restorations
    • 3: Inability to maintain the marginal tissue in periodontal health with atraumatic plaque removal techniques using a manual or mechanical toothbrush with soft bristles
    • 4: Orthodontics — planned, in progress, or completed (except canine exposure)
    • 5: Need for restorative care of the tooth
    • 6: Progression of recession
    • 7: Severe root sensitivity with recession
C - Not medically necessary: Soft tissue grafting is considered not medically necessary when the indications in A or B are not met.

Policy C is the exclusionary fallback.

Soft tissue grafting is not medically necessary when the conditions listed in policy statements A or B are not met. Per policy statement C, procedures performed without documented gingival recession with mucogingival involvement (A) or without documentation of at least one of the specified indications (B) do not meet medical necessity criteria and are excluded from coverage.

Any soft tissue grafting performed in the absence of mucogingival involvement must have documentation of at least one of the listed conditions in policy statement B (for example: root abrasion/abfraction/erosion; Class V caries or defective restorations; inability to maintain marginal tissue with atraumatic plaque removal; orthodontic treatment except canine exposure; need for restorative care; progression of recession; or severe root sensitivity with recession). If none of these conditions are documented, the procedure is considered not medically necessary.

    Provider Responsibilities and Documentation

    Documentation Required

    Benefit verification and documentation

Verify member benefits and obtain/submit required clinical documentation per the Policy Guidelines before scheduling or performing soft tissue grafting procedures. Contact Blue Cross and Blue Shield of Kansas Customer Service to confirm eligibility, coverage, prior authorization requirements, and any plan-specific limitations.

    • Confirm member benefits and any prior authorization requirements with BCBSKS Customer Service
    • Submit clinical documentation listed under Policy Guidelines (see next callout)
    • Check member contract and state/federal mandates which take precedence over Medical Policy
    Note

    Provider action required

Providers must include all required clinical findings and supporting images/radiographs when submitting claims or prior authorization requests to avoid processing delays or denials.

    • Include tooth number(s) and specific clinical measurements

    Background and Clinical Context

Soft tissue graft procedures include a range of surgical techniques—such as pedicle grafts, free soft tissue grafts, subepithelial connective tissue grafts, soft tissue allografts, and combined connective tissue/double pedicle grafts—performed to create or augment attached gingiva, deepen the vestibule, eliminate frenum pull, or provide root coverage. These procedures address gingival recession and related problems and are considered medically necessary only when the policy’s mucogingival involvement criterion or one or more of the specific indications are documented.

    Definitions

inv-10: Soft tissue graft — definition and variants
DefinitionSurgical procedures (free gingival, connective tissue, pedicle, soft tissue allograft, or combined connective tissue/double pedicle) performed to create or augment attached gingiva, deepen the vestibule, eliminate frenum pull, or provide root coverage.
Pedicle soft tissue graftA pedicle flap of gingiva raised from an edentulous ridge, adjacent teeth, or existing gingiva and moved laterally or coronally to replace alveolar mucosa as marginal tissues; used to cover exposed root or eliminate a gingival defect when root prominence allows.
Free soft tissue graftGingiva or masticatory mucosa grafted to another site, with or without root coverage; may eliminate frenum/muscle pull, extend the vestibular fornix, or correct localized gingival recession (includes donor site surgery).
Subepithelial connective tissue graftPer-tooth procedure using connective tissue dissected from a donor site and placed under a retained overlying flap at the recipient site to create/augment gingiva, obtain root coverage, eliminate sensitivity or prevent root caries, extend the vestibular fornix, augment ridges, or provide a gingival interface for restorations.

  1. Attach radiographs and intraoral photographs when they support medical necessity
  2. Documentation Required

    Required clinical documentation

For consideration of medical necessity, submit the specific clinical findings listed below. Incomplete documentation may result in denial of coverage.

    • Tooth number(s)
    • Probing depths (mm)
    • Gingival recession (mm)
    • Keratinized gingiva width (mm)
    • Presence of marginal inflammation
    • Evidence of progression of recession
    • Cervical caries or abrasion
    • Radiographs and intraoral photographs, if available
    • If autograft not feasible, documentation supporting use of Alloderm
    Denial Risk

Denial risk when indications not met

Claims for soft tissue grafting may be denied if the policy indications are not met. Soft tissue grafting is medically necessary only when gingival recession with mucogingival involvement is present, or when documented alternative indications (A–G) are met. If these criteria are not satisfied, the service is not medically necessary.

    • Denial risk if criteria A (mucogingival involvement) or B (documented alternative indications 1–7) are not met
    • Document clinical findings that directly support one or more listed indications to reduce denial risk
Allograft procedure to create or augment gingiva, with or without root coverage; may eliminate frenum/muscle pull, extend the vestibular fornix, and correct localized gingival recession without requiring a donor site.
Combined connective tissue and double pedicle graftCombined procedures used for advanced gingival recession when a single procedure is insufficient to achieve the desired outcome.
    inv-11: Mucogingival involvement — definition
DefinitionMucogingival involvement is a condition in which the probing depth progresses through the gingival complex and enters into mucosal tissue.
Clinical relevancePresence of mucogingival involvement is the required condition for soft tissue grafting to be considered medically necessary under policy statement A.
Implication for coverageIf mucogingival involvement is absent, soft tissue grafting is only potentially covered when one or more specific indications (policy B) are documented; otherwise it is not medically necessary.
