COPD Continuation Criteria
Authorization of 12 months may be granted for continuation of treatment of COPD in members 18 years of age or older when all of the following criteria are met:
This medication is not indicated for relief of acute bronchospasm or status asthmaticus. It is intended as add-on maintenance therapy for chronic eosinophil-driven conditions and should not be used for immediate rescue treatment of acute asthma exacerbations or life-threatening status asthmaticus.
For the hypereosinophilic syndrome (HES) indication, members with HES that is secondary to non-hematologic causes (for example, drug hypersensitivity, parasitic helminth infection, HIV infection, or non-hematologic malignancy) are excluded. Additionally, FIP1L1-PDGFRA kinase–positive HES is specifically excluded from the HES criteria and is not eligible for this therapy under the HES indication.
For all indications covered by this policy, the member must not receive this medication concomitantly with any other biologic drug or targeted synthetic drug for the same indication. Concurrent use with another biologic or targeted synthetic therapy for the same condition will render the request ineligible for coverage under these criteria.
Members who are receiving the medication as samples or through the manufacturer's patient assistance program are required to meet the same initial approval criteria as other members. Receipt of samples or participation in manufacturer assistance programs does not bypass prior authorization or eligibility requirements.
If a member does not meet the applicable coverage criteria outlined in this policy for the requested indication, use of this medication is considered not medically necessary and will not be approved.
Requests will be denied as not medically necessary when the member fails to meet the specified clinical criteria for the indication (for example, the COPD continuation criteria requiring documented improved COPD control and continued use of maintenance COPD treatments) or when the medication is used concomitantly with another biologic/targeted synthetic for the same indication.

Prior Authorization, Documentation, and Denial Triggers
Obtain prior authorization — indication-specific criteria and variable durations
Prior authorization is required for Nucala and specific clinical criteria must be met for each FDA indication; authorization durations vary by indication (generally 6 months for initial approvals and 12 months for continuation requests).
Follow FDA-consistent dosing and quantity limits
Approvals are limited to dosing consistent with FDA labeling and the policy's quantity limits (e.g., COPD: 100 mg every 28 days; dosing for EGPA/HES may be up to 300 mg every 28 days per policy quantity limits).
Provide full prior medication/treatment history and CRSwNP surgical/INCS history
Document prior medication and treatment trials for initial requests: include drug name, dose, frequency, and duration; for CRSwNP include prior sino-nasal surgery or systemic corticosteroid trial (within 2 years) or documentation why such therapy is not advisable.
Avoid concurrent biologics; provide smoking/vaping cessation counseling when applicable
Do not prescribe Nucala concomitantly with any other biologic or targeted synthetic drug for the same indication; counsel current smokers or vapers on cessation options where applicable.
Submit required pretreatment labs and indication-specific test results
For initial prior authorization requests, submit chart notes or medical records showing pretreatment blood eosinophil counts and prior medication history (drug, dose, frequency, duration); include indication-specific tests where required (e.g., FIP1L1‑PDGFRA for HES; nasal endoscopy/CT, Meltzer or NPS for CRSwNP).
Provide chart notes documenting clinical improvement for continuation
For continuation requests, submit chart notes or medical record documentation showing a positive clinical response to therapy, such as reduced exacerbation frequency/severity or improved symptoms/FEV1 (for COPD), or reduced relapse frequency or decreased corticosteroid use for other indications.
Show objective evidence of improved COPD control and continued maintenance therapy
For COPD continuation requests specifically, provide evidence of improved COPD control with Nucala (reduction in exacerbation frequency/severity or improved FEV1), confirm continued use of maintenance COPD treatments (LAMA‑LABA and inhaled glucocorticoid unless contraindicated), and ensure dosing will not exceed the FDA‑approved maximum.
Denial risk: not medically necessary when criteria unmet or concurrent biologic use
Requests will be denied as not medically necessary if the member does not meet the policy criteria for the requested indication or is receiving another biologic/targeted synthetic drug for the same indication.
Denial triggers — COPD continuation and concomitant biologic use
Common denial triggers include: failure to document improved COPD control for continuation (no reduction in exacerbations or no FEV1 improvement), lack of documentation of continued maintenance COPD therapy, exceeding FDA‑approved dose, or concomitant use of another biologic/targeted synthetic for the same indication.
- No chart evidence of improved COPD control (reduction in exacerbations or improved FEV1).
- Failure to document ongoing use of maintenance COPD therapies (LAMA‑LABA and inhaled glucocorticoid unless contraindicated).
- Requested dosing exceeds FDA‑approved/quantity limits.
- Concurrent biologic/targeted synthetic therapy for the same indication.
Policy Background
Mepolizumab (Nucala) is an anti–interleukin‑5 (anti‑IL‑5) monoclonal antibody indicated as add‑on maintenance therapy for multiple eosinophil‑driven disorders. The drug is intended to reduce eosinophil‑mediated inflammation in conditions such as severe eosinophilic asthma, eosinophilic granulomatosis with polyangiitis (EGPA), hypereosinophilic syndrome (HES), chronic rhinosinusitis with nasal polyps (CRSwNP), and eosinophilic‑phenotype chronic obstructive pulmonary disease (COPD). Coverage under this policy is aligned with FDA‑labeled indications and requires documentation of the eosinophilic phenotype and inadequate control on standard therapies prior to authorization.
