OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Documentation and Claim Submission Requirements

    Documentation Required

    Documentation and Claim Submission Requirements

Providers are responsible for submitting accurate and complete documentation for services performed and for submitting claims using valid, HIPAA-approved code sets. Claims must be coded according to industry-standard coding guidelines (including but not limited to Uniform Billing, AMA CPT, HCPCS, ICD-10-CM/PCS, NDC, DRG rules, and CMS National Correct Coding Initiative guidance). Drug and laboratory submissions are subject to claim review and may be adjudicated based on benefit coverage, provider contract terms, applicable medical and clinical payment and coding policies, and internal coding logic. Upon request, providers must furnish additional documentation to support the claim.

    • Submit claims using HIPAA-approved code sets (CPT, HCPCS, ICD-10, NDC, Revenue, Modifiers) and follow industry coding guidance.
    • Ensure documentation supports services billed, including medical records, lab orders, test results, and physician sign-off where required.
    • Drug and lab claims may be reviewed for benefit coverage, contract terms, policy compliance, and coding software logic — be prepared to provide supporting records.
    • Adhere to CMS NCCI edits, Local/National Coverage Determinations, and other applicable CMS guidelines when applicable.
    • Failure to provide requested documentation may result in claim denial or recoupment.
