inv-01: Admission Criteria

Covered when ALL of the following are met

Admission — core requirements: 1) The service is for a child/adolescent with a current DSM-5/ICD-10 diagnosis reflecting symptoms requiring BHIF; 2) Symptoms/functional impairments are severe and persistent and result in Danger to Self (DTS), Danger to Others (DTO), significant impulsivity with poor judgment, inability to engage in daily activities safely in a less restrictive setting, or risk of significant physical/sexual acting-out behavior; 3) Outpatient ambulatory behavioral health resources in the community do not meet treatment needs; 4) The child/adolescent does not require a level of medical or professional supervision beyond that available in a BHIF (transfer to an acute psychiatric hospital indicated if higher level needed); 5) Active treatment in a controlled environment with psychiatric oversight, 24-hour nursing presence, and continuous supervision is required to meet needs.

inv-02: Exclusionary Conditions (Admission Not Appropriate)

Exclusions: Admission should not be used primarily as an alternative to incarceration/preventative detention or to ensure community safety for primarily delinquent/antisocial behavior; should not be used as safe housing or permanency placement; not as an alternative to parental/guardian or agency responsibility; and not when less restrictive alternatives are available and not utilized.

inv-03: Continued Stay Criteria

Must meet ALL of the following

Continued stay core requirements: 1) Symptoms/functional impairments remain severe and persistent and are based on the DSM-5/ICD-10 diagnosis that precipitated or manifested since admission; 2) Effective transition planning to a less restrictive level of care has begun and additional inpatient days are expected to increase the probability of successful community integration; 3) Outpatient ambulatory resources continue to be insufficient to meet treatment needs; 4) The child/adolescent still does not require a level of medical or professional supervision beyond that provided in BHIF (transfer to acute psychiatric hospital if higher level required).

Professionally developed service plan should be revised if lack of progress is identified.

inv-04: Expected Improvement Requirement

Expected improvement: Active treatment available at this level of care can reasonably be expected to improve the psychiatric condition to facilitate discharge to outpatient care or family living at the earliest possible time; services must demonstrate documented improvement and the individual service plan should be revised as needed.

inv-05: Discharge Criteria

Discharge triggers: One or more of: 1) Treatment goals completed with sufficient symptom/behavior relief; 2) Functional capacity improved enabling safe care in a less restrictive setting; 3) Member can participate in monitoring/follow-up or caregiver availability; 4) Appropriate community services/providers/support are available; 5) No evidence continued BHIF stay would improve outcome; 6) Continued BHIF stay may risk regression or decompensation.

Discharge planning should begin at admission and include the Child and Family Team.

Admission to a Behavioral Health Inpatient Facility is excluded when it is used primarily for non-clinical purposes. Examples include use as an alternative to incarceration or preventative detention, to ensure community safety for youth whose behaviors are primarily delinquent or antisocial, as the equivalent of safe housing or permanency placement, or as an alternative to parental/guardian or agency responsibility. Admission is also excluded when it is being used in place of less restrictive alternatives that are available but not utilized.

Admissions to a Behavioral Health Inpatient Facility are not medically necessary when the placement is primarily for non-clinical reasons (for example, housing, detention alternative, or to substitute for parental/agency responsibility) rather than to treat severe psychiatric needs. Additionally, admission is appropriate only after community outpatient/ambulatory behavioral health services have failed or are insufficient to meet treatment needs; a lack of available outpatient services alone is not, by itself, sufficient for admission.

A lack of available outpatient services alone does not justify admission; the member must have symptoms or functional impairments that are severe and persistent and meet behavioral criteria (e.g., danger to self/others, significant impulsivity with poor judgment, inability to engage in daily activities safely) that cannot be managed in a less restrictive setting.

Outpatient care must have been attempted and documented as insufficient before admission is approved.
Admission is appropriate only when the child's/adolescent's psychiatric needs can only be met by specialized inpatient treatment.

Plans must comply with current standards for medically necessary covered behavioral health services and with 42 CFR.
