OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Private Inpatient Room Medical Necessity

    Private Hospital Room Medical Necessity

A private hospital room is considered medically necessary when ANY of the following circumstances applies:

    ANY of the following

    • Isolation is required

      • Isolation is required to avoid jeopardizing the member's health and/or recovery.
      • Isolation is required to avoid jeopardizing the recovery of other patients who are likely to be alarmed or disturbed by the member's condition.
    • Immediate hospitalization when only private rooms are available

      • The member needs immediate hospitalization because their medical condition is such that hospitalization cannot be deferred.
      • The facility has no semi-private or ward accommodations available at the time of admission.
    • The member is admitted to a facility that has only private accommodations and no semi-private or ward accommodations.

Use of a private hospital room primarily at the member's request, when semi-private accommodations are otherwise available, is not medically necessary.

Deluxe-accommodation charges are not medically necessary.

Private hospital room use that does not meet the qualifying medical-necessity circumstances above is not medically necessary.

Member-requested private accommodations are not medically necessary when semi-private accommodations are available.

Provider charges specifically for deluxe accommodations are not medically necessary.

    Documentation, Authorization, and Benefit Verification

    Prior Authorization

    Verify Benefits and Authorization Requirements

Verify the member’s benefits before providing a private inpatient room. Prior approval or prenotification may be required, and coverage is subject to the subscriber contract, including group-specific exclusions, mandates, and ASO or FEP provisions.

    • Contact the member’s health plan customer service department when benefits are unclear.
    • For NEHP/ABNE or FEP members, confirm applicable benefit rules through the appropriate customer service resources or plan documents.
    • Member contract or employer benefit-plan language takes precedence over this medical policy when the two conflict.
    Documentation Required

    Support Private-Room Medical Necessity

Document the clinical and facility circumstances supporting a private room as medically necessary. A private room may be supported when isolation is required to protect the member’s health or recovery, or to protect other patients who may be alarmed or disturbed by the member’s condition.

    Policy Context

A private hospital room may be medically necessary when isolation is required to protect the member's health or recovery, or to protect other patients who may be alarmed or disturbed by the member's condition. Examples include communicable disease, heart attack, cerebrovascular accident, and psychotic episodes that require isolation for certain periods. A private room may also be medically necessary when immediate hospitalization is required and no semi-private or ward accommodation is available at admission, or when the admitting facility offers only private accommodations.

    • Document the isolation rationale and relevant clinical condition; examples that may require isolation for certain periods include communicable disease, heart attack, cerebrovascular accident, and psychotic episodes.
    • Document when immediate hospitalization was medically required and semi-private or ward accommodations were unavailable at admission.
    • If the facility has only private accommodations and no semi-private or ward rooms, document that facility limitation.
    Prior Authorization

    Complete Authorization Submission

Submit complete authorization or prenotification information, including the clinical and facility documentation supporting the requested accommodation. Incomplete requests may delay the decision while missing information is obtained.

    • Include the applicable isolation rationale, urgency of admission, room availability at admission, or all-private-facility status.
    • Ensure the submission addresses the policy’s qualifying medical-necessity circumstances and contains all requested information.
    Denial Risk

    Member-Requested or Deluxe Accommodations

A private room requested primarily for the member’s preference is not medically necessary when semi-private accommodations are otherwise available. Provider charges specifically for deluxe accommodations are also not medically necessary.

    • Do not represent member-requested private-room preference as a qualifying medical-necessity circumstance.
    • Do not separately charge for deluxe accommodations as a medically necessary service.
    Denial Risk

Private Room Does Not Meet Criteria

Private-room use that does not meet a qualifying medical-necessity circumstance is not medically necessary and may be denied. Apply the criteria for required isolation, immediate hospitalization when no semi-private or ward room was available, or admission to a facility with only private accommodations.

    • A member-requested private room is not qualifying when semi-private accommodations are available.
    • Confirm that documentation supports at least one applicable medical-necessity circumstance before billing or submitting the request.
