OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medical Necessity and Coverage Criteria

    Initial therapy

Initial use may be considered medically necessary when ALL four criteria are met.

    Initial REN use

    • Age: The individual is 8 years of age or older.age >= 8 years
    • Migraine diagnosis and duration: The individual has a 6-month history of headaches meeting International Classification of Headache Disorders (ICHD 3-beta) diagnostic criteria for migraine with or without aura.headache history >= 6 months
    • Qualifying medication or clinical circumstance

      • Medication response or tolerance: Insufficient response, contraindication, or intolerance to 2 or more guideline-recommended headache medications, such as anticonvulsants, antihypertensives, antidepressants, or CGRP inhibitors.2 or more medications
      • Pregnancy or conception: The individual is pregnant, breastfeeding, or planning to conceive.
      • Medication-overuse risk: The individual is at risk for or has a history of medication-overuse headache.
      • Drug-interaction risk: The individual is at risk for drug-drug interactions with medications for comorbid conditions.
      • Pediatric medication contraindication or intolerance: The child has a documented contraindication or intolerance to a medication.
    • Covered indication

      • Acute treatment: REN is used to treat acute migraines.
      • Prevention: REN is used to prevent migraine in an individual with 6 to 24 headache days per 28-day period in each of the 3 months preceding use of the REN device.6–24 headache days per 28-day period for each of 3 preceding months

    Continuation therapy

Continued use may be considered medically necessary when BOTH criteria are demonstrated.

    Continued REN use

    • Compliance: The individual is compliant with use of the REN device.
    • Clinical benefit: The individual demonstrates clinical benefit, such as fewer migraines or shorter duration of migraines.

Lost, stolen, or destroyed DME replacement

Replacement of lost, stolen, or destroyed DME may be covered when ALL criteria are met.

    DME replacement

    • No alternative coverage: The item is not covered by an alternative entity, including homeowners or automobile insurance.
    • Health-risk consequence: Absence of the DME would put the member at risk of death, disability, or significant negative health consequences such as a hospital admission.
    • Warranty: The DME is still under warranty.

Non-FDA-approved remote electrical neuromodulation (REN) devices are considered investigational and are not covered under this policy.

REN use in individuals with contraindications is considered investigational. Contraindications include uncontrolled epilepsy and an active implantable medical device, such as a pacemaker, hearing-aid implant, or other implanted electronic device.

Trigeminal nerve stimulation devices, including Cefaly, are considered investigational for migraine treatment under this policy.

All other REN indications that do not meet the policy’s stated medical-necessity criteria are considered investigational.

Replacement of more than one lost, stolen, or destroyed durable medical equipment item, prosthetic, or orthotic per Plan Year is excluded. Replacement is also excluded when the applicable replacement criteria have not been met.
