Monitor payer policy activity
This policy governs coverage and prior authorization for IB-STIM percutaneous electrical nerve field stimulation in children and adolescents with ROME IV-defined functional gastrointestinal disorders. It establishes medical-necessity criteria, investigational nonqualifying uses, coding instructions, and related equipment-replacement rules for eligible Blue Cross Blue Shield of Vermont members.
Effective January 1, 2026, CPT code 0720T was deleted from the prior-approval coding table.
Effective January 1, 2026, CPT code 64567 was added to the prior-approval coding table.
The August 2026 policy review reported no change to the policy statement and updated references.
Medical Necessity and Nonqualifying Uses
Medically Necessary IB-STIM
The IB-STIM® device may be considered medically necessary in children and adolescents when ALL of the following criteria are met:
ALL of the following
- The patient is 11–21 years of age.
- The patient has a ROME IV-defined functional gastrointestinal disorder, including functional abdominal pain, functional abdominal pain syndrome, irritable bowel syndrome, functional dyspepsia, or abdominal migraine.
- Organic gastrointestinal disease has been ruled out.
- The problem has been present for at least 9 months.>= 9 months
- The patient has tried and failed medications in all three categories: acid suppression (H2 blockers or PPIs); antispasmodics or motility medications (hyoscyamine, dicyclomine, erythromycin, linaclotide, or prucalopride); and neuromodulators (amitriptyline, nortriptyline, gabapentin, Periactin, or aprepitant), in addition to diet modification.
Investigational Uses
All other uses of the IB-STIM® device not meeting the medical-necessity criteria above are considered investigational.
Investigational uses include any of the following
- Use in patients who are not 11–21 years of age.
- Use for a condition that is not a ROME IV-defined functional gastrointestinal disorder, including functional abdominal pain, functional abdominal pain syndrome, irritable bowel syndrome, functional dyspepsia, or abdominal migraine.
- Use when organic gastrointestinal disease has not been ruled out.
- Use when the problem has been present for less than 9 months.
- Use when the patient has not tried and failed medications in all three required categories, in addition to diet modification.
- Any other use of the IB-STIM® device that does not meet all medical-necessity criteria.
Uses of the IB-STIM® device that do not meet all of the medical-necessity criteria are considered investigational.
Replacement of lost, stolen, or destroyed durable medical equipment, prosthetics, or orthotics is not covered if the medical-necessity criteria above have not been met and the request is for more than one lost, stolen, or destroyed durable medical equipment, prosthetic, or orthotic per Plan Year.
Prior-Approval Coding
| 64567 | Percutaneous electrical nerve field stimulation, cranial nerves, without implantation. |
Authorization and Documentation Requirements
Prior Approval Required
Prior approval is required for CPT 64567, subject to applicable benefit terms and limitations. The code describes percutaneous electrical nerve field stimulation of the cranial nerves without implantation.
- CPT 64567 — Percutaneous electrical nerve field stimulation, cranial nerves, without implantation.
- Policy instruction: Prior Approval Required.
Required Treatment History
For IB-STIM® to be considered medically necessary, the patient must have tried and failed medication therapy in all three required categories, in addition to diet modification.
- Acid suppression: H2 blockers or PPIs.
- Antispasmodic or motility medications: hyoscyamine, dicyclomine, erythromycin, linaclotide, or prucalopride.
- Neuromodulators: amitriptyline, nortriptyline, gabapentin, Periactin, or aprepitant.
- Diet modification must also have been tried and failed.
Medical-Necessity Documentation
Submit documentation supporting that the patient meets the medical-necessity criteria, including eligibility, the qualifying diagnosis, exclusion of organic gastrointestinal disease, duration of symptoms, and prior treatment history.
- Document age 11–21 years.
- Document the ROME IV-defined functional gastrointestinal disorder diagnosis.
- Document that organic GI disease has been ruled out.
- Document that the problem has been present for at least 9 months.
- Document failed medications in all three categories and failed diet modification.
Document Stolen Equipment
For a replacement request involving stolen durable medical equipment, submit supporting documentation with the request, such as a police report.
- Replacement is subject to the applicable replacement criteria, including warranty status and risk of significant negative health consequences if the equipment is absent.
Nonqualifying IB-STIM® Use
Uses of the IB-STIM® device that do not meet all of the medical-necessity criteria are considered investigational.
- Nonqualifying uses are not covered as medically necessary under this policy.
Complete Authorization and Verify Benefits
Submit complete authorization information before requesting a determination. Incomplete requests may be delayed while missing information is obtained, and benefits must be verified before providing the service because coverage varies by contract and member group.
- Confirm that the authorization request addresses the policy guidelines and includes all required clinical documentation.
- Verify member benefits and any applicable exclusions with the health plan or customer service department.
- NEHP/ABNE, FEP, ASO, and other group or individual contracts may have different benefits; contract language takes precedence when it conflicts with the policy.
DME Replacement Limits
Replacement of lost, stolen, or destroyed durable medical equipment is excluded when the replacement criteria are not met or when more than one item has already been replaced during the plan year.
- No more than one lost, stolen, or destroyed item may be replaced per Plan Year.
- The item must not be covered by an alternative entity, must still be under warranty, and its absence must pose a risk of death, disability, hospital admission, or another significant negative health consequence.
Therapy Overview
Percutaneous electrical nerve field stimulation delivers electrical impulses through the external ear to cranial nerve bundles and brain areas involved in pain processing. IB-STIM is intended for pediatric patients with functional abdominal pain associated with functional gastrointestinal disorders, including irritable bowel syndrome, and is typically administered in an office setting. The policy cites randomized, double-blind, sham-controlled trials reporting reductions in abdominal pain and improvements in overall symptoms among adolescents.
