    Medical necessity criteria

    • Age: The patient is 11–21 years of age11–21 years
    • Diagnosis: The patient has a Rome IV-defined functional gastrointestinal disorder, including functional abdominal pain, functional abdominal pain syndrome, irritable bowel syndrome, functional dyspepsia, or abdominal migraineRome IV-defined disorder
    • Exclusion of organic disease: Organic gastrointestinal disease has been ruled outOrganic GI disease ruled out
    • Duration: The functional gastrointestinal problem has been present for at least 9 monthsAt least 9 months
    • Conservative treatment failure: The patient has tried and failed medications in all three categories—acid suppression, antispasmodic or motility medications, and neuromodulators—in addition to diet modificationFailure of all 3 medication categories plus diet modification

Acid suppression includes H2 blockers or PPIs; antispasmodic or motility medications include hyoscyamine, dicyclomine, erythromycin/linaclotide, or prucalopride; neuromodulators include amitriptyline, nortriptyline, gabapentin, Periactin, or aprepitant.

    Investigational uses

All other uses of the IB-STIM device that do not meet the medical-necessity criteria above are considered investigational.

Investigational use: The use does not meet all stated medical-necessity criteriaAny required criterion absent or unsatisfied

  2. Documentation Required

    Medical Necessity Documentation

Submit medical-necessity documentation supporting that the patient meets all eligibility requirements for IB-STIM®. Documentation should establish the diagnosis, exclusion of organic disease, duration of symptoms, and prior treatment history.

    • Patient is 11–21 years of age.
    • ROME IV-defined functional gastrointestinal disorder is documented, such as functional abdominal pain, functional abdominal pain syndrome, irritable bowel syndrome, functional dyspepsia, or abdominal migraine.

    • Required medication failures and diet modification are documented.
    Documentation Required

    Documentation for Stolen DME Replacement

Replacement requests for lost, stolen, or destroyed durable medical equipment, prosthetics, or orthotics must include documentation supporting the request. A police report or similar documentation is required for a stolen item.

    • Document that the absence of the equipment would put the member at risk of death, disability, or significant negative health consequences, such as hospital admission.
    • Document that the equipment is still under warranty.
    • Document that replacement is not covered by an alternative entity, including homeowners or automobile insurance.
    • Submit a police report when requesting replacement of stolen equipment.
