Defines medical necessity and coverage criteria for outpatient cardiac monitoring modalities (external loop recorders, mobile cardiovascular telemetry/MCT including Ziopatch, implantable loop recorders, CardioMEMS implantable PAP sensor) and other monitoring devices; lists devices considered experimental/investigational and durable medical equipment coverage positions. Applies to all lines of business unless overridden by Medicare/Medicaid NCD/LCD or plan documents.
