OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Coverage Criteria for Canakinumab (Ilaris)

Initial therapy — Systemic Juvenile Idiopathic Arthritis (sJIA)

Covered when ALL of the following are met for pediatric members:

Age: Member aged 2 to 17 years
Duration of disease: Persistent sJIA activity for a minimum of six (6) months; date of diagnosis must be noted
Prior therapies: Trial and failure of NSAIDs and corticosteroids for >3 months consecutively within the last 4 months (paid claims will be reviewed for verification)
Joint activity: Either ≥5 active joints with concomitant fever for at least 2 weeks within the last 3 months OR >2 active joints with concomitant fever for at least 5 days plus trial of prednisone or equivalent 0.5 mg/kg/day or 30 mg/day within the last 3 months
Inflammatory markers: CRP >15 mg/L and ESR >45 mm/hr within the last 2 months
Fever >38°C (100.4°F) for at least 2 weeks within the last 2 months
Prior biologic failure: Documented failure of Kineret (anakinra) and either Arcalyst (rilonacept) or Actemra (tocilizumab); failure defined as paid claims for at least 6 months and lack of response in labs
Exception: History of anaphylactic reaction to Kineret, Arcalyst, or Actemra or development of severe skin reactions (Stevens-Johnson syndrome)

Initial therapy — Adult Onset Still's Disease (AOSD)

Covered when ALL of the following are met for adult members:

Age: Member aged 18 to 75 years
Diagnostic features: Member must meet at least two: fever >39°C lasting ≥1 week; arthralgia/arthritis ≥2 weeks; typical rash; leukocytosis >10,000/mm3 with >80% PMNs
Disease activity: DAS28 ≥3.2 at screening
Inflammatory markers: CRP >15 mg/L and ESR >45 mm/hr within the last 2 months
Joint counts: At least 4 painful and 4 swollen joints for at least 2 weeks within the last 3 months

    Reauthorization / Continuation criteria

Covered for continued therapy when ALL of the following are met:

For sJIA: Documentation showing decreased ESR to <30 mg/L and ESR <13 mm/h and decreased number of swollen joints
For AOSD: Documentation showing decreased ESR to <30 mg/L and ESR <13 mm/h, decreased number of swollen joints, and DAS28 decreased to <2.6
Documentation submission: Refax form with documentation of CRP or ESR along with progress notes to document therapy effectiveness for continued 12-month approval; progress notes and labs documenting anaphylaxis or SJS must be submitted

Use of free or manufacturer samples to start therapy does not satisfy the plan's step-edit or prior authorization requirements. Samples will not be accepted as evidence of a trial or failure of prior therapies when verifying treatment history.

All required clinical documentation — including lab results, diagnostics, and chart or progress notes — must be submitted with the request. Authorization may be delayed or denied if the information provided is incomplete, incorrect, or illegible.

    What Providers Must Do

    Prior Authorization

Prior authorization and site‑of‑care for Ilaris (J0638)

Canakinumab (Ilaris) (HCPCS J0638) requires prior authorization; it is designated non‑preferred under the medical benefit and “medication can ONLY be provided by a Physician's office.”

    • HCPCS code: J0638
    • Non‑Preferred (Medical)
    • Site‑of‑care: must be provided by physician's office
    Step Therapy

Verification of prior therapies — claims or chart notes

Prior therapies will be verified through pharmacy paid claims or submitted chart notes; note that use of free samples to initiate therapy does not meet step‑edit/preauthorization criteria.

    • Verification via pharmacy paid claims or submitted chart notes
    • Samples used to initiate therapy do NOT meet preauthorization/step‑edit criteria

    Background

Systemic juvenile idiopathic arthritis (sJIA) and adult onset Still's disease (AOSD) are inflammatory disorders that commonly present with fevers, arthritis or arthralgia, and elevated inflammatory markers (CRP, ESR). Canakinumab (an IL‑1β inhibitor) is requested for treatment of sJIA in pediatric members and AOSD in adults when specific diagnostic features, disease activity thresholds, and prior‑therapy requirements are met.

Requests for canakinumab must document the clinical features and objective inflammation measures described in the criteria, and authorization decisions will rely on submitted labs, diagnostics, and chart notes to demonstrate disease activity and prior treatment history.

    Definitions

    inv-15: Anaphylaxis
DefinitionAnaphylaxis is defined as an emergency department (ER/ED) visit due to throat or tongue swelling and/or shortness of breath
Use in criteriaHistory of anaphylactic reaction to Kineret, Arcalyst, or Actemra is an exception in initial-therapy criteria
