OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Benlysta (belimumab) IV

    Initial Therapy

Covered when ALL of the following are met for Initial Authorization (separate paths for SLE and Lupus Nephritis):

General administrative requirements: Prescribing physician must sign form; member and prescriber information and required documentation submitted; dosing schedule and diagnosis documented; age >= 5 years for indications.
Autoantibody confirmation (require one): Member is autoantibody-positive by ONE of: ANA titer >=1:80 OR anti-dsDNA >=30 IU/mL.
Disease activity confirmation for SLE (require one): Member's SLE activity confirmed by ONE of: SELENA-SLEDAI score of 6-12 OR >=2 BILAG B organ domain scores.
Prior therapy requirement for SLE: Member has tried three of specified standard therapies and is established on two of them for the last 90 days (submit chart notes). Listed agents include: mycophenolate, hydroxychloroquine, azathioprine, cyclophosphamide, methotrexate, cyclosporine, corticosteroids, or other.
Exclusions to eligibility: Member must NOT have severe active central nervous system lupus, current or previous diagnosis of progressive multifocal leukoencephalopathy (PML), or concurrent use with other biologics.
Lupus nephritis specific criteria: For LN: prescribed by or in consultation with a Nephrologist or Rheumatologist; age >=5; active LN Class III, IV, or V confirmed by renal biopsy; autoantibody-positive SLE as above; has active renal disease and received standard therapy for 90 days (mycophenolate, calcineurin inhibitor, or cyclophosphamide); baseline urine protein measurement (uPCR or urine protein) within last 30 days.

    Reauthorization/Continuation Therapy

Reauthorization covered when ALL of the following are met:

Continuation of initial criteria: All initial authorization criteria continues to be met.
Demonstrated response for SLE: Member response confirmed by ONE of: SELENA-SLEDAI improved by and/or maintained >=4 points below baseline OR no new BILAG-A organ domain scores and no more than 2 new BILAG-B scores.
Demonstrated response for LN: Member has improvement or stabilization in urine protein:creatinine ratio or urine protein (submit labs within last 30 days).
Safety for continuation: Member has absence of intolerable side effects such as serious infections, signs/symptoms of PML, malignancy, severe hypersensitivity/anaphylaxis, or serious infusion reactions.

The member must NOT have any of the following limitations to therapy: severe active central nervous system (CNS) lupus, a current or previous diagnosis of progressive multifocal leukoencephalopathy (PML), or be receiving concurrent use with other biologic therapies. Requests with documentation of these conditions are excluded from coverage and will not meet medical necessity for Benlysta (belimumab) intravenous infusion.

Provider Actions, Prior Authorization, and Documentation

    Prior Authorization

Prior authorization required; prescriber signature and form completion

Prior authorization is required for Benlysta (belimumab) intravenous infusion when billed as J0490. The prescribing physician must sign and clearly print their name on the medical prior authorization/step-edit request form (preprinted stamps not valid).

    • Medical HCPCS code: J0490 (Benlysta IV).
    • Fax completed form to 1-877-535-1391; ensure phone and fax numbers on the form are correct to avoid delays.
    Step Therapy

Required prior/concomitant therapies and documentation of trials

For SLE, the member must have tried three of the listed standard therapies and be established on two of them for the last 90 days with chart notes documenting trials and insufficient disease control. For lupus nephritis, the member must have active renal disease and have received standard therapy for the last 90 days with chart notes documenting established therapy.

    • Examples of listed agents for SLE: mycophenolate, hydroxychloroquine, azathioprine, cyclophosphamide, methotrexate, cyclosporine, corticosteroids, or other specified agent(s).

    Background on Medication and Dosing

Benlysta (belimumab) for intravenous infusion is administered using weight-based dosing and scheduling documented on the prior authorization form. The policy references the IV dosing paradigm of 10 mg/kg given on a loading schedule (doses at 0, 2, and 4 weeks) followed by maintenance infusions every 4 weeks. The request form requires documentation of diagnosis, age (member must be ≥ 5 years), and laboratory confirmation of active, autoantibody-positive disease as part of the clinical background supporting IV therapy.

  1. For lupus nephritis, acceptable agents include mycophenolate, a calcineurin inhibitor (e.g., cyclosporine, tacrolimus), or cyclophosphamide.
  2. Submit chart notes documenting therapy trials and that therapies were taken with insufficient disease control; show establishment on required agents for the prior 90 days.
  3. Documentation Required

Required documentation to support the request

Submit the completed request form with prescriber signature and printed name, member and prescriber contact information, diagnosis and ICD code, weight (and date weight obtained if applicable), and supporting clinical documentation including lab results for autoantibody status and disease activity measures.

    • Autoantibody documentation: ANA titer ≥1:80 OR anti-dsDNA ≥30 IU/mL (submit lab results).
    • Disease activity measures: SELENA‑SLEDAI score or BILAG results as specified (submit results).
    • For lupus nephritis, submit baseline urine protein measurement (uPCR or urine protein) collected within the last 30 days (submit lab results).
    • Chart notes documenting prior therapy trials and establishment on required therapies for the last 90 days.
    Denial Risk

Incomplete or missing information may delay or cause denial

Incomplete, incorrect, or illegible information or missing required documentation (labs, diagnostics, chart notes) on the request form can delay authorization or lead to denial; all requested documentation must be provided to support each checked criterion.

    • Preprinted signature stamps are not valid — prescriber must sign and clearly print name.
    • If information provided is not complete, correct, or legible, authorization can be delayed.
    • Missing lab results (autoantibody tests, SELENA‑SLEDAI/BILAG scores, urine protein/uPCR) or missing chart notes documenting prior therapy trials may result in denial.
