    Timely Filing Reconsideration Criteria

OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Timely Filing Reconsideration Criteria

The health plan will reconsider reimbursement for claims denied for timely filing when the provider supplies proof of claim receipt within applicable timely filing limits and demonstrates Good Cause per the policy.

    ALL of the following

    Prior Authorization

Request Reconsideration for Timely Filing Denials

Provider may request reconsideration of a claim denied for late filing by supplying proof that the claim was received within the applicable timely filing period and, where applicable, evidence that ‘Good Cause’ exists. Acceptable proof for mailed claims is an official mailing service return receipt or delivery confirmation together with a mailed claims log; acceptable proof for electronic claims is the clearinghouse‑assigned receipt date from reconciliation reports. Fax transmissions and hand‑delivery items (claim log and signed receipt) are not acceptable proof.

    • Mail: official mailing service return receipt or delivery confirmation AND a mailed claims log that identifies each claim (see required mailed claims log fields).
    • Electronic: clearinghouse assigned receipt date from reconciliation reports.
    • Not acceptable: fax transmission; hand delivery (claim log and signed receipt).
    • Good Cause: provider statement/evidence will be the primary basis; plan may request clarification and considers administrative error, retroactive enrollment, incorrect member information, unavoidable delay obtaining documentation, unusual circumstances beyond provider control, or destruction/damage to provider records (unless caused by provider).
