OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Corrected Claims Reimbursement Criteria

    Corrected claims reimbursement criteria

Reimbursement for corrected claims is allowed when filing requirements are met and subject to timely filing limits and other contractual or regulatory constraints.

Corrected claim reimbursement is allowed only when the corrected claim is received within the applicable timely filing requirements of the original claim; unless provider, state, federal, or CMS contracts and/or requirements indicate otherwise.
Timely filing limit: corrected claims must be received within 365 days from the date of service for both participating and nonparticipating providers and facilities.
Corrected claims filed beyond federal, state-mandated, or company standard timely filing limits will not be approved and are not reimbursed unless the provider presents documentation proving timely filing.
Submission format requirements: paper corrected claims must be clearly marked 'Corrected Claim'; electronic corrected claims must include the applicable frequency code. Failure to mark or use the correct frequency code may result in the claim not being approved as a duplicate.
Separate submission requirement: corrected claims must be submitted separately for each member and episode of care; batch, bulk, or packaged submissions are not accepted.
Waiver authority: The health plan may temporarily waive corrected claim filing requirements following documented natural disasters or in accordance with applicable state guidance.
Coding and documentation: corrected claims must follow standard correct coding (CPT/HCPCS/revenue codes) and codes must be supported by the medical record; improper coding or billing may result in rejection, denial, recovery, or adjustment of payment.

    Corrected Claim Submission Requirements

    Billing Rule

Mark corrected paper claims or use the correct electronic frequency code

When resubmitting a corrected claim, providers must clearly mark paper claims with the words "Corrected Claim" or, for electronic submissions, use the applicable frequency code (for example, -7 for replacement or -8 for void/cancel). Corrected claims must be received within the timely filing limit and submitted separately for each member and episode of care; failure to mark or use the correct frequency code may result in the claim not being approved as a duplicate.

    • Timely filing limit: 365 days from date of service for both participating and nonparticipating providers and facilities.
    • Use frequency code -7 for Replacement of a Prior Claim or -8 for Void/Cancel of Prior Claim for electronic corrections.
    • Corrected claims filed beyond federal, state-mandated, or company timely filing limits will not be approved unless provider presents documentation proving timely filing.
    • Corrected claims must be submitted separately per member and episode of care; batch, bulk, or packaged submissions are not accepted.
