Coverage Criteria for Aristada Initio
Initial Therapy
Requests may be approved when ALL of the following criteria are met:
Aristada Initio approval criteria
- Age: Individual is 18 years of age or older>=18 years
- Diagnosis: Individual has a diagnosis of schizophreniaICD-10 F20.0-F20.9
- Oral tolerability: Individual has established tolerability with oral aripiprazoledocumented tolerability
Required prerequisite
- Initiation status: Individual is initiating or re-initiating therapy with Aristada
- Concomitant Aristada injection: Will be used in conjunction with first Aristada (aripiprazole lauroxil) injection (may be same day or up to 10 days later)<=10 days if not same day
- Oral aripiprazole dose rules: Will be used in conjunction with one 30 mg dose of oral aripiprazole for either: initiating therapy with Aristada OR re-initiating therapy after >7 weeks since last Aristada 441 mg injection or >12 weeks after other Aristada strengths30 mg single oral dose

Denial triggers (NOT approved)
- Repeat dosing: Use for repeat Aristada dosing (routine repeat use of Aristada Initio) is not approved
- Lack of oral tolerability: Individual has not established tolerability to oral aripiprazole
Requests without documented oral aripiprazole tolerability are not approved
Requests for Aristada Initio (aripiprazole lauroxil) used as a repeat dose for ongoing Aristada maintenance are not approved. Aristada Initio is intended only to initiate or re-initiate Aristada therapy and is not for routine repeat dosing.
Requests where the individual has not demonstrated tolerability to oral aripiprazole are not approved. Established tolerability with oral aripiprazole is a required prerequisite for coverage of Aristada Initio.

Provider Requirements, Prior Authorization, and Denials
Prior Authorization Required
Prior authorization is required. Requests will be approved when all clinical criteria are met, including: age ≥ 18 years, diagnosis of schizophrenia, established tolerability to oral aripiprazole, and use of Aristada Initio only to initiate or re‑initiate Aristada IM therapy in conjunction with the first Aristada injection and a single 30 mg oral aripiprazole dose.

Tolerability and Sequencing Requirements
Aristada Initio is intended ONLY for initiation or re‑initiation of Aristada IM therapy and not for routine repeat dosing. The individual must have established tolerability to oral aripiprazole prior to use of Aristada Initio. Aristada Initio plus a single 30 mg oral aripiprazole dose allows immediate transition to Aristada without the usual 21‑day oral supplementation. The first Aristada injection may be given the same day as Aristada Initio or up to 10 days afterward.

Background
Aristada Initio (aripiprazole lauroxil) is an intramuscular extended‑release injection indicated to initiate or re‑initiate Aristada long‑acting injectable therapy for schizophrenia in adults. It enables initiation or re‑initiation of Aristada without the usual prolonged 21‑day oral supplementation required by other regimens and must be used with a single concomitant 30 mg oral aripiprazole dose per labeling when starting or re‑starting therapy.
