Air Ambulance Medical Necessity

Air Ambulance Medical Necessity

Air ambulance transport, including emergency and non-emergency ambulance transportation (NEAT), may be medically necessary when one of the following alternative pathways is met. Air ambulance services must be licensed by the Louisiana Department of Health (LDH) Bureau of Emergency Medical Services (EMS); rotor-wing and fixed-wing emergency aircraft must be certified by the Bureau of Health Services Financing (BHSF) for reimbursement.

ONE of the following medical-necessity pathways

• Speed and access pathway

• Speedy admission of the member is essential.
• The member's point of pickup is inaccessible by a land vehicle.
• Distance or obstacle pathway

• Great distances or other obstacles are involved in transporting the member.
• Those distances or obstacles require transport to the nearest hospital with appropriate services.

Provider Requirements and Review

Prior Authorization

Emergency Transport Is Post-Payment Reviewed

Prior approval is not required for emergency air ambulance transportation, including mileage. Claims are reviewed after the service is delivered rather than approved before transport.

• Applies to emergency air ambulance transportation services and associated mileage.
• Do not delay emergency transport to obtain prior approval.
Documentation Required

Support Retrospective Claim Review

Emergency air ambulance claims are received and reviewed retrospectively. Providers should retain and furnish records supporting the emergency transport, medical necessity, applicable transport pathway, and regulatory compliance when requested during post-payment review.

• Support retrospective claim review with documentation that speedy admission, an inaccessible pick-up point, great distance, or other obstacles required air transport.

Air Ambulance Transportation Overview

Air ambulances may be used for emergency and non-emergency ambulance transportation when medically necessary. Air transport may be appropriate when speedy admission is essential and the pickup location is inaccessible by land vehicle, or when great distances or other obstacles make transport to the nearest hospital with appropriate services necessary. The policy addresses both rotor-wing helicopters and fixed-wing aircraft.

1. If both land and air ambulance services are necessary during the same trip, each provider is reimbursed separately under the regulations applicable to that provider type.
2. Billing Rule

Maintain Regulatory Credentials

Maintain all credentials required for reimbursement. Air ambulance services must be licensed by the Louisiana Department of Health Bureau of Emergency Medical Services under La. R.S. 40:1135.8, and rotor-wing and fixed-wing emergency aircraft must be certified by the Bureau of Health Services Financing (BHSF).

• Ensure emergency medical technicians, registered nurses, respiratory care technicians, physicians, and pilots hold personnel certifications required by applicable state laws and regulations.
• Verify that the aircraft certification and service licensure remain current.

Medical Necessity Criteria Must Be Met

Air ambulance transport may be medically necessary only when a stated clinical pathway is met. Reimbursement is at risk when the transport does not satisfy the applicable medical-necessity criteria.

• Speedy admission is essential and the member pick-up point is inaccessible by a land vehicle; or
• Great distances or other obstacles require transport to the nearest hospital with appropriate services.
Denial Risk

Required Licensure or Certification Missing

Reimbursement may be denied when required Louisiana licensure, BHSF aircraft certification, or applicable personnel certification is absent or cannot be demonstrated.

• Maintain LDH Bureau of Emergency Medical Services licensure for the air ambulance service.
• Maintain BHSF certification for rotor-wing (helicopter) and fixed-wing emergency aircraft.
• Maintain certifications required for emergency medical technicians, registered nurses, respiratory care technicians, physicians, and pilots.
