    Medical Necessity Criteria

    Bariatric surgery medical necessity

Bariatric surgery may be medically necessary when all applicable criteria are met.

    Bariatric surgery general requirements

    • Authorization and multidisciplinary evaluation: Prior authorization is obtained, and a preoperative evaluation was completed within the previous 12 months by a multidisciplinary team including, at a minimum, a physician, nutritionist or dietician, and licensed qualified mental health professional.Within the previous 12 months

For members under age 18, the multidisciplinary team must have pediatric expertise.

    • Preoperative readiness: The evaluation documents that previous weight-loss attempts were unsuccessful, future attempts other than bariatric surgery are unlikely to be successful, and the member is capable of adhering to the post-surgery diet and follow-up care.

For members capable of becoming pregnant, counseling must address avoiding pregnancy preoperatively, for at least 12 months postoperatively, and until weight has stabilized.

    • Age-specific medical-necessity pathway

      • Members age 18 and older: The member has a body mass index equal to or greater than 40 kg/m² or is more than 100 pounds overweight; or has a body mass index of 35 to 39.9 kg/m² with at least one qualifying obesity-related comorbidity; or has a body mass index of 30 to 34.9 kg/m² with type 2 diabetes mellitus and inadequately controlled hyperglycemia despite optimal oral or injectable medication.BMI ≥40 kg/m²; BMI 35–39.9 kg/m² with qualifying comorbidity; or BMI 30–34.9 kg/m² with inadequately controlled type 2 diabetes

Qualifying adult comorbidities include type 2 diabetes, cardiovascular disease, poorly controlled hypertension, a history of coronary artery disease treated with coronary artery bypass or percutaneous transluminal coronary angioplasty, cardiomyopathy, obstructive sleep apnea confirmed on polysomnography with an AHI or RDI >30, or another obesity-related comorbidity expected to improve with weight loss.

      • Members age 13 through 17: The member has a body mass index equal to or greater than 40 kg/m² or 140% of the 95th percentile for age and sex, whichever is lower; or has a body mass index of 35 to 39.9 kg/m² or 120% of the 95th percentile for age and sex, whichever is lower, with at least one qualifying obesity-related comorbidity.BMI ≥40 kg/m² or 140% of the 95th percentile; or BMI 35–39.9 kg/m² or 120% of the 95th percentile with comorbidity

Qualifying adolescent comorbidities include obstructive sleep apnea confirmed on polysomnography with an AHI >5, type 2 diabetes mellitus, idiopathic intracranial hypertension, nonalcoholic steatohepatitis, Blount's disease, slipped capital femoral epiphysis, gastroesophageal reflux disease, hypertension, or another obesity-related comorbidity expected to improve with weight loss.

    Post-bariatric panniculectomy

Panniculectomy subsequent to bariatric surgery may be medically necessary when all criteria are met.

    Post-bariatric panniculectomy requirements

    • Significant medical or functional consequence: The pannus causes cellulitis, other infections, skin ulcerations, or persistent dermatitis that has failed to respond to at least three months of non-surgical treatment; or causes functional impairment such as interference with ambulation. At least three months of failed non-surgical treatment for qualifying skin complications

At least one medical or functional consequence is required.

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for coverage of bariatric surgery, including open or laparoscopic procedures that revise gastrointestinal anatomy to restrict stomach size, reduce nutrient absorption, or both.

    Documentation Required

    Multidisciplinary Preoperative Evaluation

Submit documentation of a preoperative evaluation completed within the previous 12 months by a multidisciplinary team consisting at minimum of a physician, nutritionist or dietician, and licensed qualified mental health professional. For members younger than 18, the team must have pediatric expertise.

    • Evaluation must be completed within the prior 12 months.
    • Team composition must include a physician, nutritionist or dietician, and licensed qualified mental health professional.
    • The multidisciplinary team must have pediatric expertise for members under age 18.

    Clinical Background

Bariatric surgery revises the gastrointestinal anatomy through open or laparoscopic procedures to restrict stomach size, reduce nutrient absorption, or both. These anatomical changes support weight loss and may address obesity-related health risks when the applicable medical-necessity requirements are met.
