Supporting documentation is required when filing a claim that includes unlisted procedure codes; lack of supporting documentation will result in denial.

Unlisted codes must not be used when a more descriptive or appropriate procedure code exists; claims will be denied if a more appropriate code is available.

Unlisted procedure codes with a modifier appended are reviewed and may be denied.

When performing two or more procedures that require the same unlisted CPT code, report the unlisted code only once to identify the services provided (excludes unlisted HCPCS codes).

Additional reimbursement will not be provided for special techniques or equipment submitted with an unlisted procedure code.

If services are determined to bundle, the provider will be notified and additional payment will be denied.

If the procedure is experimental, prior authorization is required.

Documentation must be legible and include: a clear description of the nature/extent/need for the procedure; whether the procedure was independent or performed with other services (same site/opening); any extenuating circumstances; time, effort, and equipment used; number of times the procedure was performed; and designation of the portion of the report that identifies the test or procedure associated with the unlisted code.

Documentation by code category

Surgical procedures (unlisted CPTs 10021-69990): operative or procedure report required.
Radiology/imaging (70010-79999): imaging report required.
Laboratory and pathology (80047-89398): laboratory or pathology report required.
Medical procedures (90281-99607): office notes and reports required.
Unlisted HCPCS codes: operative or procedure report required.
Other unlisted CPTs (e.g., 01999, 17999, 29999): provide appropriate clinical documentation based on procedure type (operative report, imaging, office notes).

Documentation should include a description of the number of times the procedure was performed and clearly identify the portion of the documentation associated with the unlisted code.

Prior Authorization

If the procedure is experimental, obtain prior authorization before performing or billing the service. Payment determinations remain subject to the Health Plan's terms and may change according to the plan operating or administration rules.

Denial Risk

Denial risk for insufficient documentation or modifier use

Claims submitted with unlisted procedure codes without supporting documentation are subject to denial; unlisted codes appended with a modifier are reviewed and may be denied.

Provide required supporting documentation when filing claims with unlisted procedure codes to avoid denial.
Do not rely on appending modifiers to unlisted procedure codes as a guarantee of payment; such claims are subject to review and possible denial.
When multiple procedures require the same unlisted CPT code, report the unlisted code only once (excludes unlisted HCPCS).
