Coverage criteria for IV hydration

Conditions under which CPT 96360/96361 are reportable and denied:

Initial IV hydration (CPT 96360) is reportable only when duration is >= 31 minutes and <= 1 hour.

Each additional hour (CPT 96361) may be billed in addition to CPT 96360 when the additional hour exceeds 30 minutes (i.e., reportable for each additional hour beyond the initial hour if >30 minutes).

Do not report CPT 96360 or 96361 for infusions of drugs or when fluids are incidental to drug administration (fluids used to administer drugs are not separately reportable).

Do not report CPT 96360/96361 for fluids administered solely to maintain venous access (KVO/TKO); these are considered neither diagnostic nor therapeutic and should not be reported separately.

Claims billed with CPT codes 96360 and 96361 are flagged by code auditing software for clinical validation. A clinician reviews the member and provider's current and historical claims and any supporting diagnoses. Clinical validation occurs prior to claims payment. Once a claim has been validated, it is either released for payment or denied. Providers may submit medical records for reconsideration or appeal. Documentation must include a physician's order for IV hydration, the start and stop times of the infusion, and the infusion rate to support the reported duration of administration.
