OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medical Necessity and Continuation Criteria

    Initial Therapy

Home NPWT is medically necessary when all required conditions are met.

    Initial home-use eligibility

    • Wound-therapy qualification

      • Failed therapy: A complete wound therapy program has been tried and failed.
      • Contraindicated therapy: Complete wound therapy programs are contraindicated.

    Continuation Therapy

Coverage may continue only before either discontinuation condition occurs.

    Continued coverage

    • Ongoing wound response: Measurable wound healing has occurred during the prior month.Improvement in wound surface area, calculated as length times width, or wound depth
    • Treatment-duration limit not reached: Fewer than four months have elapsed using an NPWT pump for treatment of the most recent wound.Up to four months

Inpatient NPWT time before discharge counts toward the four-month maximum.

Coverage is discontinued when any measurable degree of wound healing has failed to occur during the prior month. Wound healing is measured by improvement in the wound’s surface area, calculated as length multiplied by width, or in the wound’s depth.

Coverage is discontinued when four months have elapsed using an NPWT pump to treat the most recent wound. The four-month period includes NPWT applied during the inpatient stay before discharge.

    Documentation, Eligibility, and Billing Actions

    Step Therapy

    Conventional Wound Therapy Prerequisite

Before requesting or continuing home NPWT, document that a complete wound therapy program was either tried and failed or is contraindicated, consistent with InterQual requirements.

    • Document the applicable therapy program and whether it was completed and failed or was contraindicated.
    • Do not rely on home NPWT eligibility unless one of these pathways is established.
    Documentation Required

    Medical-Necessity and Progress Documentation

The record must support medical necessity for home NPWT and show ongoing wound progress. Include documentation of the open wound, the need for NPWT during the inpatient hospital stay, the applicable complete wound therapy program, and measurable wound status over time.

    • Open wound requiring NPWT during an inpatient hospital stay.
    • Complete wound therapy program tried and failed, or contraindicated.

About Home Negative Pressure Wound Therapy

Negative pressure wound therapy (NPWT) uses a suction pump and wound dressing system to support treatment of open wounds that are low-healing or nonhealing. Under this policy, home NPWT is considered medically necessary in the specified inpatient-transition context and may continue only while measurable wound healing occurs and the maximum treatment period has not been reached.

    Treatment-Duration Tracking

Track the treatment period from the first inpatient application of the NPWT device and maintain the cumulative pump-use duration for the most recent wound.

    • Record the date of the first inpatient NPWT application.
    • Include inpatient NPWT time before discharge when calculating duration.
    • Track cumulative pump use for the most recent wound against the four-month maximum.
    Denial Risk

    Initial Eligibility Not Established

Home NPWT coverage is not established when the record does not demonstrate an open wound requiring NPWT during an inpatient hospital stay or does not establish an eligible conventional wound-therapy pathway.

    • Deny or do not continue coverage when the inpatient NPWT need is not documented.
    • Deny or do not continue coverage when a complete wound therapy program was neither tried and failed nor shown to be contraindicated.
    Denial Risk

No Healing or Maximum Duration Reached

Discontinue home NPWT coverage when no measurable healing has occurred during the prior month or when four months of NPWT pump use for the most recent wound have elapsed.

    • Measurable healing means improvement in wound surface area (length × width) or depth.
    • The four-month period includes NPWT applied in the inpatient setting before discharge home.
    Billing Rule

    Canister Billing Limit

For Medicaid claims, disposable canisters (A7000) may be billed only with the NPWT pump (E2402) and are limited to 10 canisters per 30 days.

    • Deny A7000 when billed without E2402.
    • Deny quantities exceeding 10 A7000 canisters in a 30-day period.
