Behavioral Health Treatment Record Requirements

Required documentation criteria

Behavioral health treatment records for HIM plans must contain ALL of the following elements, as applicable:

Documentation general requirements: Entries legible, dated, signed/authenticated with licensure/certification; member name on each page; date of service documented at the top of each note and on each page

Rendering provider requirements: Provider must be independently licensed in the state of service, the rendering provider on the claim must be the individual who performed the service, and services provided must be within scope of licensure; certain provisional or trainee provider types are excluded (examples: CIT, RDS, LAC, LMSW, PLP)

Clinical content requirements: Diagnosis consistent with the most current DSM and supported by presenting problems, symptom history, mental status exam, and other assessment data; type of service; exact start/stop times; reason for service; individualized service encounters; results of required screenings/assessments; support for medical necessity for frequency/intensity; treatment plans with objective measurable goals, estimated timeframes, and preliminary individualized transition/discharge plan; clear interventions linked to goals and evidence-based treatment; progress summaries; discharge summary when applicable including summary of care, condition at discharge, rationale, and written discharge instructions with referrals/follow-up/medication regimen as applicable

The policy requires that the rendering provider be an individually licensed practitioner who both performs and is billed for the service. The rendering provider must be independently licensed in the state where the service is provided, must be the same individual identified on the claim, and must provide services only within the scope of their licensure. Examples of licensed provider types include LCSW, LMFT, LP, LPEI, LPC, LADAC, APRN, MD, and DO. Conversely, certain provisional or trainee roles are explicitly listed as examples of provider types that are not acceptable as rendering providers (this list is not all‑inclusive): Counselors in Training (CIT), Rehab Day Specialists (RDS), Licensed Associate Counselor (LAC), Licensed Master Social Worker (LMSW), and Provisionally Licensed Psychologist (PLP).

Documentation that is incomplete, unclear, inconsistent, or not timely may fail to support the medical necessity of billed services. Such documentation deficiencies can create gaps in clinical communication and continuity of care, impede other practitioners from providing appropriate follow‑on services, and expose the claim to administrative action including denial of claims.

To mitigate these risks, each patient encounter should be documented completely, accurately, and in a timely manner so that the record fully discloses the services furnished and provides sufficient clinical detail to justify frequency, intensity, and type of behavioral health services billed.
