Monitor payer policy activity
This policy governs clinical documentation, service-note, treatment-activity, protocol-modification, telehealth, discharge, and related record requirements for covered Applied Behavioral Analysis (ABA) services. It applies to providers, rendering clinicians and technicians, supervising practitioners, members/enrollees, and caregivers involved in ABA services.
The policy added prior authorization, documentation-update, active-treatment, and payment-denial or recoupment requirements for ABA services.
The policy expanded service-activity-note requirements to include treatment summaries, protocol modifications, caregiver participation, telehealth safeguards, progress, treatment response, participant identification, and care coordination.
The policy added discharge-summary requirements for members/enrollees who no longer meet medical necessity criteria for ABA services.
The policy established protocol-modification documentation expectations, including a minimum of two hours per week or 10% of direct service hours, whichever is greater, and a maximum of 20% unless clinically justified.
The policy purpose was revised from providing medical necessity guidance to providing documentation guidance because the policy is not a medical necessity policy.
Former medical necessity criteria in sections I.A–I.D were removed.
Requirements were added for prior authorization approval, updated clinical documentation, and documentation and data collection.
Documentation requirements were expanded for service notes, detailed treatment-activity summaries, telehealth, progress, member/enrollee response, addenda, discharge planning, and provider and caregiver signatures.
Protocol-modification documentation requirements were added and the related code H0032 was added in multiple provisions.
Unsupported 97153 and 97154 units billed during the entire six-month authorization period may be denied or recouped.
HIPAA compliance language and additional HCPCS codes were added to the policy.
OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.
