OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    Medical Necessity Criteria for Panniculectomy

    Initial Therapy / Medical Necessity

Covered when ALL of the following are met

    Panniculectomy medical necessity

    • 1. Panniculus position: Panniculus hangs below the level of the pubis, documented by photographs

      Photographic evidence required

    • 2. Chronic complications refractory to therapy: Medical records and photographs document at least one of the following chronic and persistent complications that remains refractory to appropriate therapy for at least three months>=1 complication

Appropriate therapy includes topical antifungals, topical and/or systemic corticosteroids, and/or local or systemic antibiotics, plus good hygiene; listed complications: (a) non-healing ulceration under panniculus; (b) chronic maceration or necrosis of overhanging skin folds; (c) recurrent or persistent skin infection under panniculus; (d) intertriginous dermatitis or cellulitis or panniculitis

    • 3. Functional improvement expected: Panniculectomy is expected to restore normal function or improve functional deficit
    • 4. Weight stability when panniculus due to significant weight loss: If panniculus is due to significant weight loss, one of the following must be met

      (a) Weight loss not result of bariatric surgery and weight stable ≥ 6 months; OR (b) Weight loss due to bariatric surgery, weight stable ≥ 6 months and ≥ 18 months since surgery

A distinction is made between abdominoplasty and procedures intended to restore function. Abdominoplasty performed primarily to improve appearance is considered cosmetic and is not covered under this policy when there is no documentation of functional impairment. The policy treats abdominoplasty as a cosmetic procedure when the primary goal is aesthetic improvement rather than correction of a functional deficit.

Procedures performed solely for cosmetic reasons are not the intended covered indication under this policy. When surgery is requested only for aesthetic improvement without evidence of functional limitation, chronic complications, or other documented medical necessity, the procedure is considered not medically necessary and is not covered.

Prior Authorization, Documentation, and Clinical Requirements

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for CPT 15830 (panniculectomy). Requests must include documentation that meets the medical necessity criteria in this policy. Incomplete prior authorization requests that do not demonstrate the policy criteria are at risk for denial.

    • Prior authorization required for CPT 15830
    Documentation Required

    Required Documentation

Photographic documentation and medical records are required to demonstrate the following policy criteria. Requests missing the required photographs or documentation of chronic/persistent complications that are refractory to appropriate therapy are at high risk for denial.

    • Photographs showing panniculus hangs below the level of the pubis
    • Medical records documenting at least one chronic/persistent complication (see list below) refractory to appropriate therapy

    Key Definitions

    Definition of panniculectomy
DefinitionSurgical removal of a panniculus or excess skin and adipose tissue that hangs down over the genital and/or thigh area causing difficulty in personal hygiene, walking, and other physical activity
Functional impact describedHanging panniculus can impair hygiene, walking, and activities and predispose to intertrigo, ulcers, infection, and panniculitis
Distinction from abdominoplastyPanniculectomy is performed to restore function or treat complications, whereas abdominoplasty is usually cosmetic to improve appearance

    Background

A panniculus is excess abdominal skin and subcutaneous fat that can develop secondary to obesity and may become large enough to hang down from the body. Panniculus size is commonly described by grades (for example, grade 1 reaches the mons pubis and grade 5 extends to or past the knees). A panniculus can impair personal hygiene, walking, and other physical activities, and the skin folds beneath it may develop chronic problems such as intertrigo, non-healing ulcers, panniculitis, and recurrent or persistent infections. These functional impacts and complications are the clinical circumstances in which panniculectomy is intended to restore or improve function rather than provide a cosmetic benefit.

Medical Necessity Criteria for PanniculectomyCoding and Key ParametersPrior Authorization, Documentation, and Clinical RequirementsKey DefinitionsBackground
Evidence requiredDocumentation of stable weight in medical records
ContextApplies only when panniculus is due to significant non-bariatric weight loss
Stable weight and time since bariatric surgery — combined requirement for bariatric patients
Combined bariatric requirementWhen weight loss is due to bariatric surgery: weight must have been stable for at least 6 months AND it must be at least 18 months since the bariatric surgery
Documentation neededMedical records showing date of bariatric surgery and documented weight stability for >= 6 months
ApplicabilityRequirement applies only if panniculus is a result of prior bariatric surgery
    Documentation Required

    Documentation of Chronic/Persistent Complications

At least one of the following chronic or persistent complications must be documented in the medical record and shown to be refractory to appropriate conservative therapy for a minimum of three months:

    • Non-healing ulceration under the panniculus
    • Chronic maceration or necrosis of overhanging skin folds
    • Recurrent or persistent skin infection under the panniculus
    • Intertriginous dermatitis, cellulitis, or panniculitis
    Note

    Conservative Therapy Requirement

Conservative/appropriate medical therapy must have been attempted and documented for at least three months prior to request. Appropriate therapy includes topical antifungals, topical and/or systemic corticosteroids, and/or local or systemic antibiotics, along with good hygiene practices. Documentation should describe specific treatments tried, dates, and clinical response.

    • Topical antifungals
    • Topical and/or systemic corticosteroids
    • Local or systemic antibiotics
    • Good hygiene practices
    • Therapy tried for at least 3 months with clinical notes documenting refractory status
    Denial Risk

Documentation and Criteria Gaps — Denial Risk

Requests that lack required photographs (showing the panniculus below the pubis), do not document at least one qualifying chronic/persistent complication, or do not show that conservative therapy was attempted for the required duration are subject to denial. Ensure prior authorization submissions include clear photographs, dated clinical notes, and treatment history to avoid delays or denials.

    • Missing photographs = denial risk
    • No documentation of chronic/persistent complication = denial risk
    • No documentation of ≥3 months of conservative therapy = denial risk
