OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    • Failure of at least 2 susceptible IDSA-recommended antibiotics; or
    • Culture and susceptibility results showing resistance or lack of susceptibility to at least 1 agent from each class: trimethoprim-sulfamethoxazole, a fluoroquinolone, and a beta-lactam

    • Treatment received must be fewer than 10 days
    • The total treatment period must not exceed 10 days
    • Document positive clinical response
    • A dose increase must remain ≤ 2,400 mg (8 tablets) per day

    Insufficient Alternative-Antibiotic History

A request may be denied when the record does not demonstrate failure of susceptible alternative antibiotics or the required culture-supported resistance pathway.

    • Document failure of ≥ 2 IDSA-recommended antibiotics to which the isolated pathogen is susceptible; or
    • Document resistance or lack of susceptibility to at least 1 agent in each required class: trimethoprim-sulfamethoxazole, fluoroquinolone, and beta-lactam
