Coverage Criteria for Trigger Point Injections

Initial Therapy

Covered when ALL of the following are met

Initial Diagnostic/Stabilization: 1) Member has myofascial pain that has persisted for more than three months causing tenderness and/or weakness, restricting motion and/or causing referred pain when compressed; 2) Member has failed ≥ 3 weeks of conventional multidisciplinary medical therapy including: (i) chiropractic, physical therapy, or prescribed home exercise program or inability to tolerate such therapy (injection may be used as a bridge to therapy); (ii) NSAID unless contraindicated or not tolerated; (iii) activity modification; 3) Trigger points have been identified by palpation; 4) Trigger points are located in a few discrete areas and are not associated with widespread areas of muscle tenderness (e.g., fibromyalgia); 5) Injections are not used as the sole method of treatment but are intended to facilitate mobilization to allow non-invasive modalities (e.g., physical therapy) for muscle strengthening, flexibility, and functional restoration; 6) Up to two sets of injections at least seven days apart may be given for diagnosis and stabilization for the same trigger point; when a given body region is injected it will be considered one injection service regardless of number of injections.

Continuation Therapy / Additional Sessions

Additional injections: A) Prior injections resulted in ≥ 50% pain relief with functional improvement for ≥ 6 weeks; B) There was return of pain and/or deterioration following ≥ 6 weeks of improvement; C) Injections are given at least two months apart for up to 12 months from the initial injection (maximum of six total sessions); D) Injections are not used as the sole method of treatment but are intended to facilitate mobilization to allow non-invasive modalities (e.g., physical therapy) for muscle strengthening, flexibility, and functional restoration; when a given body region is injected, it is considered one injection service regardless of number of injections.

Unproven / Not Supported Indications

Not supported interventions: Dry needle stimulation of trigger points; trigger point injection with saline or glucose; use of Botox during trigger point injections.

Current evidence does not support these uses

The policy does not support certain trigger point techniques due to insufficient evidence. Specifically, dry needling (needle insertion without injection), trigger point injection with saline or glucose, and the use of botulinum toxin (Botox) during trigger point injections are not supported. Providers should avoid billing these approaches as covered trigger point injection services under this policy and follow the defined coverage criteria for supported procedures.

Step Therapy

Conservative Therapy Requirement

Conservative, multidisciplinary therapy must be tried and documented for at least 3 weeks prior to diagnostic/stabilization trigger point injections. Therapies should include physical modalities and medications unless contraindicated; injections may be used as a bridge when such therapies are not tolerated.

Denials may result if documentation is insufficient. Common denial triggers include lack of documented conservative therapy, absence of palpation-confirmed trigger points, evidence of widespread muscle tenderness (e.g., fibromyalgia), use of injections as sole therapy, or use of non-supported techniques (dry needling, saline/glucose injections, or Botox for trigger points).

Background

Trigger points are hyperirritable foci in taut muscle bands that can produce local and referred pain. Trigger point injections with local anesthetic, with or without corticosteroid, are performed both diagnostically and therapeutically to relieve myofascial pain and to facilitate mobilization so patients can engage in non-invasive treatments such as physical therapy. This policy frames injections as an adjunct — not as sole therapy — and permits up to two sets of injections at least seven days apart for diagnosis/stabilization, with additional sessions subject to continuation criteria.
