OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

Coverage Criteria for Genetic Testing of Hereditary Hearing Loss

GJB2/GJB6 sequencing and/or deletion/duplication analysis or multigene panel analysis — medically necessary

Covered when ALL of the following are met:

Initial diagnostic testing for hereditary hearing loss: Member/enrollee has hearing loss AND there is no known acquired cause of the hearing loss (e.g., TORCH infections, bacterial infection, age-related or noise-related hearing loss).

Applies to GJB2 and GJB6 sequencing and/or deletion/duplication analysis or multigene panel analysis to establish a diagnosis of hereditary hearing loss.

    Not supported indications

Other indications: Current evidence does not support GJB2/GJB6 sequencing and/or deletion/duplication analysis or multigene panel analysis to establish a diagnosis of hereditary hearing loss for all other indications.

Requests outside the specified criteria (for example, testing when there is no documented hearing loss or when an acquired cause is established) are generally not supported by current evidence and may be denied.

Testing to establish a diagnosis of hereditary hearing loss is covered only when the request is for an individual who has documented hearing loss and when there is no known acquired cause for that hearing loss. Examples of acquired causes that must be ruled out include TORCH infections (Toxoplasma gondii, other agents, rubella, cytomegalovirus, herpes simplex virus), bacterial infection, and age-related or noise-related hearing loss. Requests submitted to establish a genetic diagnosis when an acquired etiology is already identified are not supported by current evidence and may be denied.

GJB2 and GJB6 sequencing and/or deletion/duplication analysis, or a multigene hearing-loss panel that includes these genes, is considered medically necessary to establish a diagnosis of hereditary hearing loss when the member has hearing loss and no known acquired cause. The American College of Medical Genetics and Genomics (ACMG) recommends a tiered approach and, unless a specific genetic etiology is suggested by clinical or family history, initiating comprehensive hearing-loss gene panel testing.

When ordering testing, providers may choose targeted sequencing and deletion/duplication analysis of GJB2 (connexin 26) and GJB6 (connexin 30) or opt for a broader multigene panel. Documentation should support the choice of test (for example, phenotype or family history suggesting a particular gene vs. nonspecific sensorineural hearing loss where a panel is preferred). Pre-test and post-test genetic counseling is recommended and should be documented.

    Provider Actions, Authorization & Documentation

    Prior Authorization

Obtain prior authorization for listed genetic testing CPT codes

Prior authorization is implied for genetic testing services using the CPT codes listed (81252, 81254, 81430, 81431, 81479); providers should obtain authorization per plan procedures and reference the Concert Platform for registered tests.

    • Common billing CPT codes referenced in this policy: 81252, 81254, 81430, 81431, 81479
    • See the Concert Platform for additional registered tests and authorization details.
    Step Therapy

    Step therapy not specified

No step therapy requirements are specified in this policy for GJB2/GJB6 sequencing, deletion/duplication analysis, or multigene hearing-loss panels.

    Background and Rationale

Approximately 65% of prelingual hearing loss in developed countries is genetic; the DFNB1 locus, which includes GJB2 and GJB6, accounts for a substantial proportion of autosomal recessive nonsyndromic hearing loss. Because DFNB1 contributes significantly to nonsyndromic hereditary hearing loss, testing strategies commonly include GJB2/GJB6 analysis as part of initial genetic evaluation or within comprehensive hearing-loss gene panels.

Clinical guidance endorses differentiating acquired causes (for example, TORCH infections, congenital cytomegalovirus, noise exposure, and presbycusis) from genetic causes before pursuing genetic testing. When no acquired cause is identified, sequencing and deletion/duplication analysis of GJB2/GJB6 or multigene panel testing is an appropriate next step in the diagnostic workup.

    Definitions

    DFNB1 locus (GJB2/GJB6)
    Locus componentsDFNB1 includes GJB2 (encodes connexin 26) and GJB6 (encodes connexin 30).
Role in nonsyndromic hearing lossDFNB1 accounts for an estimated ~50% of autosomal recessive nonsyndromic hearing loss and 15%–40% of deafness across populations.
Clinical implicationBecause DFNB1 contributes substantially to nonsyndromic hereditary hearing loss, GJB2/GJB6 testing is a common initial target when no acquired cause is identified.
Testing recommendationMultigene hearing-loss panels or genomic testing are recommended unless clinical/family history suggests a specific genetic etiology; DFNB1 genes remain key components of such panels.
    Genetic counseling
DefinitionPre-test and post-test genetic counseling facilitates informed decision-making, addresses the possibility of secondary or incidental findings, and establishes a plan for returning results.

Coverage Criteria for Genetic Testing of Hereditary Hearing LossCoding and Billing CodesProvider Actions, Authorization & DocumentationBackground and RationaleDefinitions
    Documentation Required

Provide documentation of hearing loss, exclusion of acquired causes, and genetic counseling

Document the presence of hearing loss and evidence that an acquired cause has been ruled out; also document pre-test and post-test genetic counseling.

    • Documented hearing loss must be present to support testing.
    • Document evidence ruling out known acquired causes (e.g., TORCH infections, bacterial infection, age-related or noise-related hearing loss).
    • Record pre-test and post-test genetic counseling that addresses potential incidental findings and plans for returning results.
    Denial Risk

Requests outside specified indications may be denied

Testing requests that do not meet criteria — for example, tests to establish a genetic diagnosis when an acquired cause of hearing loss is known or when hearing loss is not documented — are at risk for denial.

    • Medical necessity is limited to members with hearing loss and no known acquired cause (e.g., TORCH infections, bacterial infection, age-related or noise-related hearing loss).
    • Requests outside these specified indications are generally not supported by current evidence and may be denied.
Pre-test roleDiscusses purpose and potential outcomes of testing, potential incidental findings, and obtains informed consent before testing is performed.
Post-test roleExplains test results, clinical implications for the member and family, addresses incidental findings, and guides follow-up care or additional testing as needed.
