OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    POLICY UPDATE CHANGES

Added age criterion 'Member/enrollee is ≥ 18 years of age' and specified 'diabetic or idiopathic' gastroparesis in indications.

Changed wording for symptom requirement to 'Chronic intractable (drug refractory) nausea and vomiting'.

Added 'dietary modifications' to required trial therapies and included FDA HDE specifications as a requirement.

Added several HCPCS billing codes (L8685, L8686, L8687) to code tables.

Coverage Criteria for Gastric Electrical Stimulation

    Initial therapy / Indications

GES is medically necessary for diabetic and idiopathic gastroparesis when ALL of the following are met:

Primary criteria: A. Member/enrollee is ≥ 18 years of age; B. Diagnosis of diabetic or idiopathic gastroparesis confirmed by gastric emptying scintigraphy; C. Chronic intractable (drug refractory) nausea and vomiting; D. Documented intolerance or failure of a trial of antiemetic, dietary modifications, and prokinetic drug therapy; E. Not currently pregnant; F. Technology is provided in accordance with the Humanitarian Device Exemption (HDE) specifications of the U.S. Food and Drug Administration (FDA).

    All elements required

Not medically necessary / Unsupported indications

GES is not medically necessary for the following:

Not covered indications: Reduction of pain, fullness, bloating, or acid reflux symptoms; any other indications not supported by peer-reviewed literature, including treatment of obesity.

Explicitly listed as not medically necessary or unsupported

Use of gastric electrical stimulation (GES) for indications other than diabetic or idiopathic gastroparesis is not supported by current peer‑reviewed evidence. This explicitly includes use of GES for the treatment of obesity and other non‑gastroparesis indications, which the policy identifies as unsupported.

GES is considered not medically necessary when used with the goal of reducing pain, fullness, bloating, or acid reflux symptoms. The policy states there is no evidence to support efficacy of GES for these symptom targets, and such uses should not be approved as medically necessary.

Provider Actions, Prior Authorization, and Documentation Requirements

    Prior Authorization

    Prior Authorization Required

Prior authorization is required for implantation and related procedures billed with the listed CPT/HCPCS/C-codes. Submit documentation demonstrating the member meets all policy clinical criteria (age ≥18, diagnosis confirmed by gastric emptying scintigraphy, chronic intractable drug-refractory nausea/vomiting, intolerance or failure of prior therapies, not pregnant, and HDE use per FDA specifications).

    • Affected CPT codes include: 43647, 43648, 43881, 43882, 64590, 64595, 95980, 95981, 95982.
    • Affected HCPCS/Codes include: C1767, C1778, L8679, L8680, L8685, L8686, L8687, L8688.
    Documentation Required

    Required Prior Conservative Therapy

Approval requires documentation of a documented intolerance or failure of conservative therapies prior to GES. Record trials of antiemetic therapy, dietary modification, and prokinetic therapy (current recommended prokinetic combination includes metoclopramide and erythromycin; centrally acting antidepressants may be used as symptom modulators).

    Regulatory Definitions and Device Status

HDE / Humanitarian Use Device (HUD)
DefinitionHumanitarian Use Device (HUD): a device intended to benefit patients in the treatment or diagnosis of a disease or condition that affects fewer than 8,000 individuals in the United States annually.
Regulatory pathwayHumanitarian Device Exemption (HDE) is the FDA mechanism that authorizes HUDs for clinical use under specified conditions.
Institutional oversight requirementHUDs may only be used in facilities that have an established institutional review board (IRB) to supervise clinical testing of devices.
Local approval requirementUse of a HUD requires approval by an IRB or an appropriate local committee for the specific disease or diagnostic use.
Policy implication for coverageTechnology must be provided in accordance with the FDA HDE specifications as a required element for medical necessity of GES implantation.

Background on Gastroparesis and Gastric Electrical Stimulation

Gastroparesis is a disorder of delayed gastric emptying in the absence of mechanical obstruction, typically presenting with nausea, vomiting, early satiety, bloating, and abdominal discomfort. Common etiologies include idiopathic and diabetic causes. Gastric electrical stimulation involves implantation of electrodes into the gastric muscularis connected to an implanted pulse generator; clinical evidence supports symptom improvement for nausea and vomiting and some quality‑of‑life measures, though improvement in gastric emptying is not consistently demonstrated.

    Denial Risk

    Triggers for Denial

Requests will be denied if required documentation is missing or criteria are not met. Common triggers for denial include: no gastric emptying scintigraphy confirming diabetic or idiopathic gastroparesis; absence of chronic intractable (drug‑refractory) nausea and vomiting; lack of documented intolerance or failure of antiemetic, dietary, and prokinetic therapy; member under 18 years of age; current pregnancy; or failure to document HDE-consistent use per FDA/IRB requirements.

    • No scintigraphy confirmation of gastroparesis.
    • Insufficient documentation of chronic, drug‑refractory nausea and vomiting.
    • No documented trials or failures/intolerance to antiemetic, dietary, and prokinetic therapies.
    • Member age < 18 years.
    • Member currently pregnant.
    • Use not documented in accordance with HDE/IRB requirements.
HDE designation reflects devices intended for conditions with very small patient populations and carries specific use restrictions and oversight.
