OpenPayer is powered by Trek Health's payer performance platform. Trek continuously ingests, validates, and normalizes Transparency in Coverage data alongside payer policies and other commercial payer data to create a structured payer intelligence foundation. OpenPayer uses this foundation to deliver personalized search results, dynamically generated policy pages, and tailored policy monitoring based on each user's payers, specialties, billing codes, and areas of interest. The same intelligence powers broader payer performance workflows, including reimbursement benchmarking, contract evaluation, payer negotiations, and financial decision-making.

    ANY of the following

    • Member has physical, intellectual, or medical conditions that make dental treatment under local anesthesia unsafe and potentially ineffective.
    • Member demonstrates uncooperativeness, fear, anxiety, or inability to communicate, with dental needs of such severity that postponing treatment would compromise the member's/enrollee's health.

    Denial Risk

    Document a qualifying medical indication

Requests for general anesthesia or intravenous (IV) sedation may fail medical-necessity criteria when none of the policy’s qualifying clinical circumstances is present. Document at least one qualifying indication: age younger than six years with complex dental treatment; a physical, intellectual, or medical condition making local anesthesia unsafe and potentially ineffective; severe dental needs with uncooperativeness, fear, anxiety, or inability to communicate such that postponement would compromise health; or oral-facial and/or dental trauma for which local anesthesia would be ineffective or compromised.

    • Age younger than six years with a dental condition requiring complex treatment.
    • Physical, intellectual, or medical conditions that make treatment under local anesthesia unsafe and potentially ineffective.
    • Uncooperativeness, fear, anxiety, or inability to communicate, with dental needs so severe that postponing treatment would compromise health.
    • Oral-facial and/or dental trauma for which treatment under local anesthesia would be ineffective or compromised.

    Clinical Background

General anesthesia and IV sedation may facilitate the safe and humane delivery of dental diagnostic and surgically invasive procedures for a limited subset of patients. This includes children who are cognitively immature, highly anxious or fearful, have special needs, or are medically compromised and unable to receive treatment in a traditional office setting.

Dental sedation and anesthesia may be provided in nontraditional settings, including acute-care inpatient facilities and ambulatory surgery centers. Providers must be appropriately qualified and trained for the intended level of sedation or anesthesia, and locations administering general anesthesia must have emergency drugs, resuscitation equipment, and properly trained staff capable of responding to anesthetic emergencies.
